The Health Information Management (HIM)  Coordinator completes a variety of duties related to the timely and accurate review, retrieval and release of medical information. This includes but is not limited to; answering and directing incoming phone calls, sorting and logging of all HIM mail, and applying the proper pre-established hospital, departmental, and/or legal policies and procedures to each request for release of medical record information.

One year or more years experience in an HIM environment preferred.

Job duties include but not limited to:

* Processes incoming requests for release of medical record information in a timely manner and consistently verifies validity of request and authorization.

* Diligently reviews requests for proper authorization, expired releases, correlation on dates of service.

* Competently reviews medical record for identification of specific protected information (including HIV related information and substance abuse documentation).

* Proactively handles the receipt and distribution of subpoenas. Accurately validates the patient, dates of service, and over-all appropriateness of the subpoena.

* Effectively coordinates and assists HIM Team Leader with various projects performed on a monthly basis. .

* Assists customers with the functionality of the patient portal (MyHealthChart). Proactively troubleshoots to resolve issues and resets passwords or initiates new access/key codes, as necessary.

* Retrieves medical records for outpatient services which include, but are not limited to; Community Memorial Hospital Family Health Centers.
