Our network of family health centers, proudly affiliated with Community Memorial Hospital, is dedicated to delivering comprehensive, compassionate care for individuals and families across our community. Each center is staffed by experienced healthcare professionals-physicians, nurse practitioners, nurses, and support staff-who share a commitment to personalized, patient-centered care close to home.
Conveniently located throughout the region, our Family Health Centers offer a full range of primary care services, including preventive care, routine checkups and annual wellness visits, pediatric and adolescent care, adult primary care, women’s health, immunizations, chronic disease management, and health education.
By integrating advanced medical technology with Community Memorial’s network of specialists, we provide seamless coordination of care when additional services are needed. Our goal is to make high-quality healthcare accessible, affordable, and tailored to the unique needs of every patient and family we serve.
Effective 8/3/26, for eligible Medicare patients, we also offer a care management program, ChartSpan, which extends support beyond the exam room. This program helps patients stay healthier between office visits with remote access to a 24/7 care line staffed by nurses and clinicians, personalized care plans and care goals, assistance scheduling appointments and managing prescriptions, and connections to valuable community resources such as transportation services. By providing ongoing support, care management can help reduce unnecessary emergency room visits, hospitalizations, and overall healthcare costs. If you’re eligible, a program representative may contact you to discuss enrollment, or you can ask your provider for more information.
Whether you need an annual physical, ongoing management of a chronic condition, or urgent care for sudden illness, our Family Health Centers provide timely, coordinated care in a welcoming environment. We accept most insurance plans and offer extended hours at select locations to accommodate busy schedules.
