Important Information about Paying for Your Care

Community Memorial Hospital (CMH) is a participating provider in many health plan networks. You can find a list of the health plans in which CMH participates here. Some health plans use smaller networks for certain products they offer so it is important to check whether CMH participates in the specific plan you are covered by. You should also understand that your particular health plan may not cover 100% of the cost of the visit/procedure and that you may be responsible for out of pocket costs, and these may vary by visit/procedure and facility. We encourage patients to contact their health plan provider to confirm the visit/procedure will be covered and to estimate their out of pocket cost. You may contact CMH’s patient financial assistance office by calling (315) 824-6552 and they can assist you in estimating the out of pocket cost that may be associated with your visit/procedure.

It’s also important for you to know that the physician services you receive in the hospital are not included in the hospital’s charges. Physicians who provide services at the hospital may be independent voluntary physicians or they may be employed by the hospital. Physicians bill for their services separately and may or may not participate in the same health plans as the hospital. You should check with the physician arranging your hospital services to determine which plans that physician participates in. CMH contracts with a number of physician groups, such as anesthesiologists, radiologists and pathologists, to provide services at the hospital. Contact information for the physician groups the hospital has contracted with is available here. You should contact these groups directly to find out which health plans they participate in.

You should also check with the physician arranging for your hospital services to determine whether the services of any other physicians will be required for your care. Your physician can provide you with the name, practice name, mailing address and telephone number of any physicians whose services may be needed. Your physician will also be able to tell you whether the services of any physicians employed or contracted by CMH are likely to be needed, such as anesthesiologists, radiologists and pathologists. Hospitals are required by law to make available information about their estimated standard charges for the items and services they provide. Please note that not all hospitals have the same charges and that not all hospitals provide the same quality of care. It is important to weigh both these factors when choosing a hospital. Updated Visitor PolicyEffective Monday, September 13, 2021

Visiting hours will be strictly enforced. Exceptions may be allowed on a case by case basis with leadership approval.

Visiting hours will be as follows for Med/Surg and SCU:

  • Monday through Friday: 3pm – 7pm
  • Saturday & Sunday: 1pm – 4pm
  • Each patient will only be allowed a maximum of TWO visitors per day.
  • NO visitors in Amb/Surg.
  • Visitors must maintain social distancing of six feet, even in patients rooms.
  • Children under the age of 12 will not be allowed to visit.
  • Visitors are not permitted to visit and patient who is in isolation, unless exceptional circumstances and prior approval have been obtained.
  • Visitors are to remain in the patient room at all times unless directed by staff to leave due to a treatment.
