PLEASE NOTE:

Please bring ID that reveals date of birth and admission ticket. To expedite registration, a consent form will be emailed within 24 hours of the event. We ask that you complete this prior to presenting for vaccine. If you are unable, copies will be available at event to complete.

Please plan to stay 15 minutes after your vaccine for observation. The MODERNA vaccine requires 2 vaccines and therefore, you will be scheduled for your 2nd dose on 5/13/2021 prior to leaving the event.

REGISTRATION System works BEST if using laptop, desktop or iPad with Google Chrome/Mozilla Firefox. Registering on phone usually times you out at DOB.
