Community Memorial enters next phase of $31.5 million transformation project.

The Community Memorial Hospital (CMH) is excited to enter the second phase of its $31.5 million Transformation Project of its facility that will expand and modernize patient care spaces.

After securing financing with a USDA guaranteed loan, Community Memorial Hospital (CMH) is excited to resume its project to transform over half of the Hospital. “The Transformation Project at Community Memorial will modernize our facility and upgrade our equipment, elevating the level of care, improving clinical workflows and increasing efficiencies across the Hospital,” stated Jeff Coakley, President and CEO of CMH.

“With support from the USDA, federal, state and local officials, we are now able to move forward and make good on our promise to renew our hospital.”

The first phase, completed in 2021, transformed the Inpatient Wing and the Emergency Department. The new Inpatient Wing brought all-new, modern single-patient rooms, offering patients and their visitors a comfortable and private setting to get well. The new ED took an outdated, small care space to an expanded unit with private patient treatment bays, complete with a dedicated registration and waiting room space.

To date, this year has been a whirlwind of progress. We’ve been busy extending our reach and expertise, bringing new specialty services to the New Hartford and Cazenovia markets. Perhaps the most exciting development is the official continuation of construction on our transformative $31.5 million upgrade of our hospital. This project will revolutionize how we deliver healthcare, offering a modern, comfortable environment equipped with the latest technology.

We recently celebrated a major milestone with the successful completion of Phase I of our Foundation’s capital campaign. Through your incredible generosity, we’ve raised $4.1 million to advance our Transformation Project. As we move into Phase II, your continued support will be instrumental in achieving our ambitious vision to be the healthcare provider and employer of choice in the region.

Community Memorial enters next phase of $31.5 million transformation project continued

The new inpatient wing opened in January 2021 as part of Community Memorial’s Phase I portion of the transformation project.

Phase I was completed just in time to meet the needs of the community during the height of a regional COVID spike. Since opening the new Emergency Department, CMH has seen record numbers of patients that need care. Phase II aims to bring ancillary services to the forefront, updating space and equipment.

The Radiology Department will be centralized inside the hospital to meet the needs of our referred patients and allow staff to conveniently respond to the diagnostic needs of our ED providers. Aging equipment will be upgraded with state-of-the-art medical imaging devices, including a CT scanner, MRI machine, and digital mammography.

The Laboratory Department will have expanded waiting space for patients with updated draw rooms, improving the patient experience. Staff will also have improved workflow and efficiencies with the expanded space.

The updated Center for Rehabilitation at CMH, which includes physical and occupational
therapies, will be expanded, offering larger treatment spaces. New and modern equipment will enhance the patient experience and care to promote recovery and mobilization, and a Patient Lounge will be added for short-term rehabilitation patients to socialize with loved ones.

Lastly, Phase II will feature a new Main Entrance for patients, families, and visitors to access the Hospital and enhance patient safeguards. The new main entrance will connect to outpatient services of radiology, lab, and therapy services with stronger patient security services and a larger waiting area.

Community Memorial and construction management group, The Hayner Hoyt Corp., anticipate completion of the project in the first quarter of 2026. While construction is in process, the organization will work vigorously to ensure patient care is not reduced or impacted, and it looks forward to welcoming those needing care into all locations and services.

Community Memorial Expands Primary Care Services with New Practice

The Community Memorial Hospital (CMH) network has extended its footprint for patients locally by adding another primary care location to the healthcare system.

Hamilton Medical Services, an affiliate of the Community Memorial network, has acquired a primary care practice located at 803 Sherrill Road in Sherrill. The new location joins the network’s existing primary care offices. It will offer exceptional care by Dr. Matthew McKay, who specializes in family medicine and has extensive experience in hospital medicine, serving as a hospitalist at Community Memorial for several years.

After graduating from St. George’s University School of Medicine and completing his family medicine residency at Mountainside Hospital, McKay has served as a private practice primary care provider. As an Oneida native, McKay has filled roles including the President and Vice President of a medical staff group as well as the Chairperson of the Department of Internal Medicine before joining Community Memorial. “I’ve been honored to provide care for our patients in the inpatient setting of CMH,” remarked McKay. “Now, I look forward to getting back to my roots of primary care, extending the services the CMH team offers and collaborating with my colleagues in the outpatient settings.”

The Sherrill office previously offered care by Dr. Robert Burdick, a longtime primary care provider. “Dr. Burdick’s decision to retire has offered the opportunity for Community Memorial to grow services in the region,” stated Jeff Coakley, President and CEO. “We welcome Dr. Burdick’s patients into the CMH family and look forward to Dr. Matt McKay providing them with the same exceptional care.”

With continued recognition of the important need for primary care and the challenges faced with access to care in rural areas, CMH has dedicated considerable efforts to bolster its provider team and support staff. This concerted activity around preventative care has supported the network in several areas of growth, including primary and specialty care.

Community Memorial offers primary care in five other locations besides Sherrill (Hamilton,Cazenovia, Morrisville, Munnsville, and Waterville) and warmly welcomes patients for all services, including specialty care.

Community Memorial Expands Orthopedics Team

The Community Memorial Hospital (CMH) network continues to meet the growing demand for specialty patient care with the addition of a new physician assistant to the orthopedics team.

JACKIE IRELAND, PA-C, works alongside Dr. Kamaljeet Banga in both
the outpatient and operating room settings. Ireland joins the CMH team with over a decade of experience, with specialized support in orthopedic spine surgery, holds certifications with the National Commission on Certifications of Physician Assistants, and is professionally affiliated with the American Academy of Physician Associates (AAPA) and the NYS Society of Physician Assistants (NYSSPA).

Graduating from Le Moyne College with a Bachelor of Science in Biology and also a Master’s of Science in Physician Assistant Studies, Ireland has worked in Central New York in a busy orthopedics and spine surgery setting. This experience has lent her the skills, knowledge, and compassion to become a skilled physician assistant and now a member of the Center for Orthopedics at Community Memorial.

With a philosophy of care centering on the patient as a whole person, Ireland remarks, “I chose to join the Community Memorial network because of the close feel of the hospital and orthopedics group. I look forward to providing collaborative care with a practice that offers a variety of sub-specialties and superior orthopedic care.”

Ireland comes to the CMH family as a new location for the orthopedic practice was just opened. Situated on the hospital campus as the new Center for Specialty Services building at 160 Broad Street, the Center for Orthopedic Services moved here in early 2024 to fit the need for expanded patient care space along with a growing team of providers.

Recognizing the important need for local orthopedic care, CMH has dedicated considerable efforts to bolster its provider team and support staff. This concerted activity has led to an increase in volumes, necessitating the expansion of the clinical facility. The Center for Orthopedic Services showcases the expertise of the growing orthopedics team.

“Our medical staff welcomes Jackie Ireland as an esteemed member of our team, bringing expertise and a passion for orthopedic care,” states Michael Walsh, DO, Chief Medical Officer. “She provides a great skill set in education and knowledge with the goal of collaborative community-based care.”

With the Hospital located next door to the orthopedic practice, services are streamlined for patient efficiency. The Center for Orthopedics also offers a walk-in clinic open Monday through Friday, 8am – 1pm, and grants quick appointments following an Emergency Department visit. Community Memorial Holds Celebration Event

The Community Memorial Foundation hosted a formal celebration event Friday, June 14, 2024, in Hamilton to introduce the second phase of the Hospital’s physical transformation project.

The celebratory affair was held at the Hall of Presidents on the Colgate University campus and brought together organization leaders, medical staff, community members and supporters to highlight the accomplishment of the first phase of the hospital’s transformation project and share what’s to come for the organization with construction now resuming. Phase II will include an expanded and relocated Radiology Department featuring a dedicated Women’s Imaging Suite, a state-of-the-art Laboratory Department, an expanded Rehabilitation Department, and a brand-new Main Entrance for patients and visitors alike.

“We were excited to partner with Colgate University as lead
supporters of Community Memorial and host this event to celebrate
our success and showcase the promising future of the hospital,”

remarked Jeff Coakley, President and CEO. “A night of celebration for our hospital brought together community members in a fun and formal setting. Welcoming guests and supporters to celebrate our success was a great time.”

With dinner and dancing to music provided by the band Grand Central Station, the evening offered an exciting opportunity for attendees to socialize and gain deeper insights into the upcoming physical transformation of the hospital. “Celebrating the success of the completed inpatient wing and all-new Emergency Department while bringing our community members and supporters together is our collective initiative,” added Coakley. “Campaign Co-Chairs Mary Brundage and Dr. Bob Delorme welcomed guests to the evening of celebration, and we appreciate everyone attending.”

Hamilton Hearts

In her 70s, Erin Dineen was no stranger to hospitals. A liver transplant survivor from 15 years ago, Erin also beat breast cancer. On top of all that, she also had a valve in her heart replaced about seven years ago.

Erin had her regular cardio check-ups with Dr. Avneet Singh at the Hamilton Heart Center of Community Memorial, where it was learned that the valve previously replaced was starting to close, and Dr. Singh advised she prepare for another bypass procedure.

In the summer of 2023, Erin was home with her private nurse who noticed her skin color was concerning, and called 911. Taken by ambulance to the new Emergency Room of Community Memorial in Hamilton with difficulty breathing, Dr. Singh came immediately to meet Erin in the ER, and advised she be transferred to a cardiac catheterization lab for evaluation and treatment. “I don’t remember too much of what happened and in what order,” noted Erin. “The experience was frightening and it felt like things happened so quickly. I do remember Dr. Singh being in the ER and discussing my best treatment plan, which put me at ease.”

Cardiac catheterization labs offer treatment for heart or blood vessel problems, such as clogged arteries. A thin, hollow tube called a catheter is guided through a vessel to the heart and this procedure gives medical providers important details about the heart muscle and valves. Taken to St. Joseph’s in Syracuse, Erin needed a coronary artery bypass graft, a surgery that involves taking a blood vessel from another part of the body and attaching it to the coronary artery above and below the current blockage.

Community Memorial welcomes the New Year and looks forward to continuing to grow high-quality healthcare services in our region. 2023 was a transformative year for our healthcare system. We opened a new orthopedic practice, expanded our primary care team, set a record of Emergency Department visits, and developed a new strategic plan. All these milestones are a testament to the commitment of our board of directors, medical staff, and employees.

To start the new year off strong, we are excited to introduce the Center for Women’s Health Services and welcome Dr. Scott Beattie and Amy Roberson, PA (previously Medical Arts Gynecology and Infertility) to the Community Memorial family in January. Their gynecology and fertility specialties will certainly enhance the network of services available at Community Memorial.

In the development of this new service line, we are using a captive professional corporation, Hamilton Medical Services, PLLC (HMS). This practice allows CMH to expand its services and continue to grow to meet the needs of our community. As you see HMS throughout the region, rest assured that you will continue to receive the highest quality of care that you have come to expect from Community Memorial.

Finally, we are proud of the development of our new strategic plan, a result of collective efforts from over 200 participants in our strategic planning meetings. Providers, employees, and community members all voiced their feedback, perceptions and hopes for Community Memorial. In partnership with the Rural Health Redesign Center, a federally funded organization aimed at helping rural healthcare systems thrive, the admin team established 1–3-year goals to establish a roadmap to the success of Community Memorial.

Community Memorial Primary Care Offices Receive National Recognition

The Community Memorial (CMH) network is excited to announce that each of its five Family Health Center locations, offering primary and specialty care services, have been recognized by a national group as patient-centered medical offices, for delivering primary care and managing the full spectrum of healthcare needs.

The New York State Department of Health collaborated with the National Committee for Quality Assurance (NCQA), the creator of the patient-centered medical home (PCMH) program in developing a transformation model that encompasses all eligible primary care providers in New York State. The NYS Patient-Centered Medical Home (PCMH) Program’s goal is to expand access to high-performing primary care, which drives improved health and wellness in the communities served.

The PCMH model of care enables CMH teams to put the focus on the patient, creating a symbiotic relationship across the care continuum. The providers and staff in the family health centers strive to provide every patient with quality care and a great patient experience. “This accreditation recognizes the dedicated efforts of the Family Health Centers team and their commitment to providing quality improvement with a patient-centered approach to care,” stated Maria Grice, Vice President of Physician Services. According to the NCQA website, the PCMH model ‘…focuses on care coordination, population health, evidence-based guidelines and effective use of health information technology to meet patients’ needs’. The CMH Family Health Centers worked to improve the patient experience, deliver improved preventative care, and collaborate with other clinicians for patient care to identify care gaps.

All primary care practices that have earned recognition through NCQA have made a commitment to improving primary care by promoting the Triple Aim: better health, lower costs and better patient experience.

After about an entire month at St. Joe’s for her heart issue, Erin went to a short-term rehabilitation center for another three months for her recovery. It wasn’t until late fall 2023 that Erin was able to return home, and then to the Center for Cardiology at Community Memorial for outpatient cardiac care.

A common post-op complication developed, atrial fibrillation, or AFib for short, which is typically monitored and cared for with medication. An abnormal heartbeat, AFib is caused by extremely fast and irregular beats from the upper chambers of the heart. With regular check-in visits with Dr. Singh, Erin is happy to be on the mend and return to her everyday activities.

Working alongside Dr. Singh are two nurse practitioners, Shelby Sharp and Sherry Loucks, who both contribute to the collaborative care provided to those needing cardiac services. Bringing over two decades of expertise in both inpatient and outpatient cardiology care, Sherri Loucks has also garnered valuable experience in hospital medicine during her tenure as a Hospitalist in the Utica region. Loucks holds an impressive academic background, earning her Masters of Science in Nursing from SUNY Health Science Center in Syracuse, NY. Furthermore, she obtained her Family Nurse Practitioner certification from Community-General Hospital in Syracuse and graduated with a Bachelor of Science in Nursing from Utica College of Syracuse University.

Shelby Sharp received her Bachelor of Science in Nursing and Master of Science in Nursing with Family Nurse Practitioner from LeMoyne College. Sharp comes from a background of work in cardiology settings, and has worked within the Community Memorial primary care team since 2019. She continues to support her patient panel there, but offers additional and valuable skillsets to the Cardiology Department.

Together, the team at the Hamilton Heart Center offers outpatient care for adults including pre-operative consults and care following heart attack, surgery and other heart conditions. With diagnostic testing and treatments provided right in Hamilton, the Cardiology Department works with patients and their primary care provider to meet every facet of cardiac care needs.

Orthopedic Practice Expansion

CMH welcomed three new practitioners to its expanding orthopedic practice, collaborating for patient care as team members with Dr. Richelle Takemoto, who joined the organization in early 2023.

DR. KAMALJEET BANGA, MD, FRCSC, ABOS, FAAOS Joined The Practice As A Specialist In Orthopedics And Sports Medicine. Dr. Banga’s career has been marked by a strong background in sports medicine, upper extremity, military, and trauma care in both university and community healthcare settings.

“I am truly excited to become a part of the Community Memorial team. It is my privilege to join such a dedicated group of healthcare professionals who share a common goal of delivering exceptional orthopedic care,” stated Dr. Banga. “I am eager to bring my expertise to benefit our patients and contribute to the continued growth and success of Community Memorial’s Orthopedic Center.”

Dr. Banga is certified by the medical boards in the United States, Canada, and India. He holds board certification in sports medicine and orthopedics from the respective medical associations of these nations. Dr. Banga completed his fellowship in Orthopedic Surgery, Upper-Extremity, and Sports Medicine at McMaster University in Ontario, Canada.

“We are thrilled to welcome Dr. Banga to our Orthopedic Center at Community Memorial. His expertise in upper extremity orthopedic care and sports medicine perfectly complements our existing team,” commented Dr. Michael Walsh, Chief Medical Officer at Community Memorial. “With Dr. Banga’s arrival, we are positioned to provide our patients with even more specialized care and enhanced treatment options.”

FRANK J. WALCZAK IV, PA-C, ATC, CSCS, joins as a Physician Assistant with a foundation in orthopedics and athletic training and will work alongside Dr. Richelle Takemoto. Walczak received a Master of Science in Physician Assistant Studies from Le Moyne College in Syracuse, NY, and a Bachelor of Science in Kinesiology with a concentration in Athletic Training from the State University of New York at Cortland. Mr. Walczak holds multiple national certifications, including a NCCPA Certified Physician Associate (PA-C) certification, NSCA Certified Strength and Conditioning Specialist (CSCS), and NATABOC Certified Athletic Trainer (ATC).

DR. NICHOLAS QANDAH, NEUROSURGEON, now brings his expertise to Community Memorial. Board-certified and fellowshiptrained, he’s and a graduate of the University of Michigan, earned his medical degree from Midwestern University and following his Neurosurgery Residency at Virginia Tech Carilion Clinic, he completed the prestigious AO Complex Spine Fellowship in a dual Orthopedics and Neurosurgery program at the University of Washington-Harborview. Dr. Qandah commenced his postresidency career as an Attending Neurosurgeon and Professor of Neurosurgery at Virginia Tech Carilion Clinic, where he held roles of Director of the Complex Spine Neurosurgery Program and Assistant Residency Director.

Dr. Qandah’s pain management and minimally-invasive spine surgery expertise further extends the range of services for patients in our community.”

With over 15 years of Neurosurgical experience, Dr. Qandah’s comprehensive approach covers various neurosurgical needs, including cervical and lumbar spine care, spinal injuries, scoliosis treatment, and brain trauma. In 2018, Dr. Qandah’s significant contributions led to the establishment of CNY Brain and Spine Neurosurgery in New Hartford, NY, a practice that has flourished under his guidance.

With the new team, CMH required additional patient care space to accommodate the growing practice. With this, a state-of-the-art orthopedic practice is slated to open on the hospital campus in the former Mid-York Building, which is scheduled for early 2024. This facility will also host other visiting specialists, as the Community Memorial Leadership Team actively recruits other essential providers to meet the diverse needs of patients.

General Surgeon Adds Venous Ablation

Dr. Gregory Ripich, Community Memorial General Surgeon, has expanded services offered locally through the addition of a new procedure for varicose veins.

Vein ablation, a treatment to close off varicose veins from within the vein, is a minimally invasive outpatient procedure. Varicose veins can cause pain, heaviness, and swelling and amongst other symptoms, can be cosmetically unappealing to patients.

Diseased veins allow blood to pool, leaving visible varicose veins. Dr. Ripich inserts a catheter into a diseased vein to close it, which re-routes blood to other healthy veins. Affecting more than 30 million people in the United States, varicose veins are twisted and enlarged veins most commonly in the legs, and Dr. Ripich and the team at Community Memorial are excited to offer this relief treatment locally.

Gregory Ripich, MD, is a general surgeon with board certification from the American Board of Surgery. Dr. Ripich attended SUNY Health-Science Center in Syracuse, NY, and completed his residency and training at St. Joseph Mercy Hospital in Ann Arbor, MI. With office visits in both the Cazenovia and Hamilton Family Health Centers of Community Memorial, Dr. Ripich is welcoming patients for both medically necessary and cosmetic cases.

Community Memorial Expands Women’s Health Services with Addition of Gynecology Practice

THE COMMUNITY MEMORIAL (CMH) NETWORK IS THRILLED TO ANNOUNCE its continued growth through the acquisition of a well-established private gynecology practice in Utica, NY.

Dr. Scott Beattie and his team from Medical Arts Gynecology and Infertility, P.C. have joined forces with the Community Memorial healthcare network, solidifying their commitment to providing vital women’s health services in the existing office on Old Burrstone Road in Utica and now also in Hamilton. The longtime dedicated practice will transition to Hamilton Medical Services Center for Women’s Health.

Board-certified by the American Board of Obstetrics and Gynecology, Dr. Beattie graduated with a dual Bachelor of Arts in Biology and Chemistry from SUNY Oswego. He pursued his medical degree at SUNY Upstate Medical University in Syracuse and completed his residency in obstetrics and gynecology at the University of Vermont Medical Center. Beattie specializes in minimally invasive and advanced laparoscopic surgery, offering a wide range of gynecological services for women.

“Dr. Beattie’s decades of experience in comprehensive women’s healthcare make him a valuable addition to the CMH medical staff,” remarked Dr. Michael Walsh, Chief Medical Officer. “We eagerly anticipate collaborating on patient care in both Utica and Hamilton, expanding our commitment to providing exceptional care.”

Joining CMH alongside Dr. Beattie is Physician Assistant Amy Roberson. A New York native, Roberson earned her Bachelor of Science in Biology from Elizabethtown College in Pennsylvania and her Masters of Science in Physician Assistant studies from LeMoyne College in Syracuse. With over a decade of collaboration with Dr. Beattie, Roberson excels in delivering high-quality care to patients.

Exceptional Orthopedic Care Made More Accessible

Community Memorial (CMH) in Hamilton continues to expand the orthopedic services provided to the community, offering a new service; a walk-in orthopedic clinic. Dr. Richelle Takemoto will welcome walk-ins each Tuesday and Thursday from 9am – 1pm at her office on the CMH campus effective November 6th.

Dr. Takemoto is a board-certified orthopedic surgeon who completed fellowship training in Orthopedic Trauma. She specializes in comprehensive fracture care, post-traumatic reconstruction, and total joint arthroplasty of the shoulder, knee and hip. In February 2022, after completing extensive training and certification, Dr. Takemoto became the first female surgeon in the world to use the VELYS™ Robotic-Assisted Solution and developed a successful robotic surgery program at CMH. Dr. Takemoto will continue to expand this service as part of the CMH team.

“With no appointment required, patients are able to take advantage of the accessible but vital orthopedic care offered right at Community Memorial,” noted Dr. Michael Walsh, Chief Medical Officer. “Our community and surrounding areas will benefit from the immediate care for sprains, minor fractures and sports-related injuries, with streamlined access to medical imaging in the hospital and follow-up appointments with our Orthopedic program.”

The walk-in clinic will treat patients of all ages as Dr. Takemoto is experienced in the care of children and adults. Staff will assist in coordinating follow-up care at the time of discharge for those patients that may need it. Any patient with a life-threatening injury such as open fracture, excessive bleeding, head or spine injuries should utilize an Emergency Room for treatment.

Community Memorial Q&A

Take a look at some of our frequently asked questions!

1. If I am picked up by ambulance for transport, can I choose which hospital to go to?

EMS personnel are trained to evaluate care needs for patients and can advise on the facility in which patients may need to go to – and, the level of care provided by different facilities is a factor, also. That being said, patients do have the right to request Community Memorial as their hospital of choice and they can let their EMS transport team know this is their preference.

2. If I visit the Emergency Room of Community Memorial, will my primary care provider know?

Yes. When you register as a patient and let our team know who your primary provider is, we are able to securely send your visit information in an effort to streamline care both in the ER and for follow-up. RHIO (Regional Health Information Organization) HealtheConnections includes more than 10,000 physicians in New York State and supports the dedicated health information exchange between healthcare providers.

3. My doctor ordered lab work / medical imaging, can I choose where I go for care?

Yes! Both our Radiology and Laboratory Departments offers outpatient services to patients, with some not even requiring an appointment. Whether the ordering medical provider is part of our CMH medical team or not, we welcome any and all patients. Primary care providers, urgent care locations, specialty care providers, including surgeons, either provide patients with a hardcopy ordering requisition, or can electronically send orders. We have the capability to send imaging results to any medical professional, so we invite patients to utilize the services provided right here in Hamilton. Often, patients may get a more convenient appointment time for their testing when choosing Community Memorial. To make an appointment with the Radiology Department of CMH, please call (315) 824-6180. To let your medical provider know you would like your testing done right in Hamilton, ask them to send the order to fax number (315) 824-6534. Our Laboratory Department blood draws are offered Mon through Fri, 8am-6:30pm but accommodations may be made for additional appointments. 4. I’m having trouble accessing my medical chart (patient portal) online. Who do I contact or call?

Patients that visit the Hospital for service will have a MyHealthChart account, and patients that utilize our Family Health Centers will have a separate patient portal referred to as ‘Health Center Records’. Both programs offer vital visit and treatment information and we encourage patients to utilize both. Our dedicated staff in both our Health Information Department (HIM) or the Family Health Center Team can assist patients with patient portal issues, including access, navigation and password reset needs. 5. I’m coming to Community Memorial for surgery. Can my family come with me or visit me?

With COVID restrictions lifted, Community Memorial is excited to welcome back visitors for our patients. Please note the following visitation availability for specific departments, to accommodate our patients and their recovery: Medical/Surgical and Special Care Departments: Open visitation is available from 7am-9pm, Monday through Friday. No one under age 12 unless approved by the charge nurse on duty. Operating Room/Recovery Room: No visitors in these areas. Emergency Department: 2 visitors per patient at a time, all other family and visitors are to remain in the waiting room.

6. I have a question about my bill, who do I contact?

Our Hospital Billing Department (along with the Medical Records Department) have relocated to 2498 Rt. 12B Hamilton, across from the new Stewart’s and Den Kelly Auto. Questions about a hospital bill may be referred in-person to the above address or by calling (315) 701.5627. Family Health Center bill questions should go to 315.937.3015. Who can I contact regarding my experience at Community Memorial? Larry Baker Family Library

Driven by personal motivation, Larry Baker established the Larry Baker Endowment Fund for Behavioral Health in 2014 and in 2019, the first project funded by the endowment was instituted; Mr. Baker is proud to now offer the Larry Baker Family Library within the Hamilton Family Health Center of Community Memorial. Located in the waiting room, there are books available for both adults and children related to mental health topics. These resources are tools for parents to learn more about experience, struggles, or ongoing behavioral and mental health issues their children may be facing.

“Each year, with the interest earned from the Larry Baker Endowed Fund for Behavioral Health, we’re able to purchase books to re-stock the Larry Baker Family Library shelves,” noted Maria Grice, Vice President of Primary Care. “These resources have been so useful for our patients that we find the books are depleted rather quickly, leaving the shelves empty.”

In an effort to keep the shelves stocked for more to utilize the resources and their information provided, Community Memorial is opening the opportunity for donations of both new and used books related to behavioral health topics. “We’ve had a local retired psychologist donate some of her books for patients and family members to take advantage of and found this is a great way for donors to recycle material they have at home and have it put to good use,” added Grice.

“We hope that the Larry Baker Family Library will encourage conversations about mental health in the context of primary care, and will provide resources for families to use to help their children manage common concerns like anxiety, depressed mood, and hyperactivity,” stated Dr. Jennifer Meyers, Pediatrician at the Hamilton Family Health Center.

Mr. Baker is thrilled the library is available and used to such a great extent, and hopes that it serves as one more support method for local families. “I’m so pleased the books are being put to such good use; now, we’d like to make sure the shelves stay stocked for even more patients and family members to have access to,” said Mr. Baker.

Women’s Health at Community Memorial

The fall season means many things, but one consistent and important part is the annual recognition and awareness month for breast cancer. Each October, healthcare providers work to raise awareness about the impact of breast cancer and also to remind patients to get their annual screening exams.

“Education is the starting point,” stated Dr. Jocelyn Morin, Family Practice Provider of the Community Memorial Family Health Center in Cazenovia. “Once patients understand and are more aware of their personal risk factors along with signs and symptoms, they can stay up to date on preventative screening needs.” According to the American Cancer Society, when detected early and in the localized stage, breast cancer has a 5-year relative survival rate of 99%. Early detection means patients do monthly self-exams of their breasts and schedule regular clinical breast exams and mammograms. “Women of all ages are encouraged to stay up-to-date on their self-exams and talk to their doctor right away if they notice anything abnormal,” added Dr. Morin.

At Community Memorial in Hamilton, a ‘fast-track’ system is in place for mammography patients, offering direct access to a dedicated waiting space after a simple registration. Certified technicians have years of specialty imaging experience, providing patients with compassionate, personalized healthcare. Comprehensive 3D-mammography services are available, with state-of-the-art ultrasound and mammography machines. Same-day sonograms for post-mammography needs are also available in some cases.

Coming Next: The ongoing transformation project will bring a renovated and expanded Radiology Department, relocated next to the new Emergency Department. This larger space will also feature a new Women’s Imaging Suite, designed with the patients in mind, offering a new and private waiting space dedicated to those patients getting a mammogram. Cardio Care

In the midst of 2022, Marney Rowland, age 53, was an avid walker, logging anywhere between four to seven miles daily. The tread on her sneakers showed her success, and the scale did, too, as she’d begun losing weight and feeling healthier. Dropping 25 pounds, Marney noticed her heart felt like it was skipping beats.

Community Memorial Primary Care Physician, Dr. Kerri Taylor, adjusted some of Marney’s medications – but the condition worsened. When she began to see some kaleidoscope colors in the corners of her eyes, Dr. Taylor referred her to Dr. Avneet Singh, Cardiologist at Community Memorial. “I had seen a cardiologist about six years ago but hadn’t had any abnormal findings,” noted Marney. “These symptoms and everything that came after was unexpected.”

In early November, Dr. Singh’s office performed an EKG (electrocardiogram) on Marney – a test recording the heart’s electrical signal to check for different heart conditions. After seeing some concerning results, a second EKG was performed immediately following. The next day, the office called Marney and asked her to return for a nuclear stress test, which uses an injection and an imaging machine to create pictures of the blood flow to the heart. Marney was then sent to Crouse Health in Syracuse for a heart catheterization, a procedure in which a thin tube is guided through a blood vessel into the heart.

“The doctor came in and said I had blockages and needed a five-way bypass, and I was sent to Upstate Hospital, next door, right away,” added Marney. “They kept me a few days, and then I had surgery. Seven hours in the OR for what ended up being a quadruple bypass, then five more days as an inpatient, and I went home.”

Post-op, Marney has some fluid still on her lungs, and her care team adjusts her medications and monitors her blood levels weekly. “Dr. Singh advised I will most likely be on A-Fib medication for a couple of months, which is common,” stated Marney. “I feel better than I did yesterday but not as good as I’ll feel tomorrow.”

Home recovering, Marney notes the appreciation for her long-time primary care provider. “I’ve seen Dr. Taylor for 20 years, I love her – and now, I’ve got a great Cardiologist with Dr. Singh. He has a great attitude with patients and always lifts me mentally.” Originally from the Oneida and Sherrill area, Marney moved north a few years ago, now traveling for an hour to see Dr. Taylor and Dr. Singh. “When you have a great medical provider you love, it’s worth going the distance to them.”

Avneet Singh, MD, FACC

Cardiologist

Dr. Singh is a Fellow of the American College of Cardiology and is certified by the American Board of Internal Medicine (ABIM) in Internal Medicine and Cardiovascular Disease. He is also certified by The National Board of Echocardiography (NBE), The Certification Board of Nuclear Cardiology (CBNC), and The Certification Board of Cardiovascular Computed Tomography (CBCCT). He is welcoming new patients at the Hamilton Heart Center.

Rebecca Dorn, FNP-C

Nurse Practitioner, Cardiology

Dorn earned her Associate’s degree in Applied Science in Nursing from Fulton Montgomery Community College and her Bachelor of Science Nursing degree from SUNY Plattsburgh. She also obtained her Master of Science in Nursing as a Family Nurse Practitioner from SUNY Upstate Medical University in Syracuse. Dorn has more than 30 years of nursing experience, most recently specialized in providing urgent care services. She has also provided care to patients as a Hospitalist and previously served as an Emergency Department Director. She is welcoming new patients at the Hamilton Heart Center.

CEO Update

Welcome to the New Year!

2022 was undoubtedly a trying year for our providers and staff as we adjusted to this new pandemic era of healthcare services. This environment created some of the most significant staffing challenges of our time while we courageously served historical volumes of patients in our Emergency Department. These patients from our community also came to us requiring a higher level of care, which challenged our hospital as well as other regional medical centers that we rely on to accept our most critically ill patients when they needed advanced services. Caring for our patients is our number one priority, and we remain committed to increasing services to patients in this community.

In 2023 we brought back our February Go Red cardiac health program with Colgate University. This event featured our Cardiology team; Dr. Avneet Singh and Rebecca Dorn, FNP-C. Also coming up, we will host a free health clinic with General Surgeon Dr. Gregory Ripich for community members to get more information on hernias, prevention and treatment, as well as a free hernia screening. It is also important to CMH that we provide critical access to primary care through urgent care and same-day services in our five Family Health Centers.

Our ongoing transformation project will resume this year as well. Construction kicked off in 2019, with plans to modernize over half of our facility. While we completed the construction of our new Inpatient wing and Emergency Department, we were ultimately caught in the downstream impact of the global pandemic. Labor and supply costs have challenged the project, and the economy has continued to drive up financing costs. Community Memorial considered all options for moving this project forward. After months of consulting and consideration, we are excited to look forward to making good on our promise to the community to transform our facility and medical equipment to match the quality care that our team provides.

I want to acknowledge the hard work and dedication of our Community Memorial Auxiliary, a team of volunteers who have supported our team in many ways. Heavily impacted by the pandemic, their ability to host their fundraisers and perform their cherished volunteer activities in the hospital was drastically reduced. Thus, they persevered and continued to do everything they could to support CMH, and they had an incredible turnout at 2022’s December Holiday Sale.
