Community Memorial enters next phase of $31.5 million transformation project.
The Community Memorial Hospital (CMH) is excited to enter the second phase of its $31.5 million Transformation Project of its facility that will expand and modernize patient care spaces.
After securing financing with a USDA guaranteed loan, Community Memorial Hospital (CMH) is excited to resume its project to transform over half of the Hospital. “The Transformation Project at Community Memorial will modernize our facility and upgrade our equipment, elevating the level of care, improving clinical workflows and increasing efficiencies across the Hospital,” stated Jeff Coakley, President and CEO of CMH.
“With support from the USDA, federal, state and local officials, we are now able to move forward and make good on our promise to renew our hospital.”
The first phase, completed in 2021, transformed the Inpatient Wing and the Emergency Department. The new Inpatient Wing brought all-new, modern single-patient rooms, offering patients and their visitors a comfortable and private setting to get well. The new ED took an outdated, small care space to an expanded unit with private patient treatment bays, complete with a dedicated registration and waiting room space.

As we turn the page on a busy and exciting first half of 2024, Community Memorial is thrilled to share some updates with you – our dedicated patients and neighbors.
To date, this year has been a whirlwind of progress. We’ve been busy extending our reach and expertise, bringing new specialty services to the New Hartford and Cazenovia markets. Additionally, the highly anticipated opening of our new Family Health Center in Sherrill provides convenience for residents seeking comprehensive care.
Perhaps the most exciting development is the official continuation of construction on our transformative $31.5 million upgrade of our hospital. This project will revolutionize how we deliver healthcare, offering a modern, comfortable environment equipped with the latest technology.
We recently celebrated a major milestone with the successful completion of Phase I of our Foundation’s capital campaign. Through your incredible generosity, we’ve raised $4.1 million to advance our Transformation Project. As we move into Phase II, your continued support will be instrumental in achieving our ambitious vision to be the healthcare provider and employer of choice in the region.

Community Memorial enters next phase of $31.5 million transformation project continued
Phase I was completed just in time to meet the needs of the community during the height of a regional COVID spike. Since opening the new Emergency Department, CMH has seen record numbers of patients that need care. Phase II aims to bring ancillary services to the forefront, updating space and equipment.
The Radiology Department will be centralized inside the hospital to meet the needs of our referred patients and allow staff to conveniently respond to the diagnostic needs of our ED providers. Aging equipment will be upgraded with state-of-the-art medical imaging devices, including a CT scanner, MRI machine, and digital mammography.
The Laboratory Department will have expanded waiting space for patients with updated draw rooms, improving the patient experience. Staff will also have improved workflow and efficiencies with the expanded space.
The updated Center for Rehabilitation at CMH, which includes physical and occupational
therapies, will be expanded, offering larger treatment spaces. New and modern equipment will enhance the patient experience and care to promote recovery and mobilization, and a Patient Lounge will be added for short-term rehabilitation patients to socialize with loved ones.
Lastly, Phase II will feature a new Main Entrance for patients, families, and visitors to access the Hospital and enhance patient safeguards. The new main entrance will connect to outpatient services of radiology, lab, and therapy services with stronger patient security services and a larger waiting area.
Community Memorial and construction management group, The Hayner Hoyt Corp., anticipate completion of the project in the first quarter of 2026. While construction is in process, the organization will work vigorously to ensure patient care is not reduced or impacted, and it looks forward to welcoming those needing care into all locations and services.

Community Memorial Expands Primary Care Services with New Practice
The Community Memorial Hospital (CMH) network has extended its footprint for patients locally by adding another primary care location to the healthcare system.
Hamilton Medical Services, an affiliate of the Community Memorial network, has acquired a primary care practice located at 803 Sherrill Road in Sherrill. The new location joins the network’s existing primary care offices. It will offer exceptional care by Dr. Matthew McKay, who specializes in family medicine and has extensive experience in hospital medicine, serving as a hospitalist at Community Memorial for several years.
After graduating from St. George’s University School of Medicine and completing his family medicine residency at Mountainside Hospital, McKay has served as a private practice primary care provider. As an Oneida native, McKay has filled roles including the President and Vice President of a medical staff group as well as the Chairperson of the Department of Internal Medicine before joining Community Memorial. “I’ve been honored to provide care for our patients in the inpatient setting of CMH,” remarked McKay. “Now, I look forward to getting back to my roots of primary care, extending the services the CMH team offers and collaborating with my colleagues in the outpatient settings.”
The Sherrill office previously offered care by Dr. Robert Burdick, a longtime primary care provider. “Dr. Burdick’s decision to retire has offered the opportunity for Community Memorial to grow services in the region,” stated Jeff Coakley, President and CEO. “We welcome Dr. Burdick’s patients into the CMH family and look forward to Dr. Matt McKay providing them with the same exceptional care.”
With continued recognition of the important need for primary care and the challenges faced with access to care in rural areas, CMH has dedicated considerable efforts to bolster its provider team and support staff. This concerted activity around preventative care has supported the network in several areas of growth, including primary and specialty care.
With the Hospital located in Hamilton, services are streamlined for patient efficiency
across all primary care offices, including ancillary services such as laboratory, radiology, rehabilitation and more. Patients and providers looking to make an appointment with any primary care office are encouraged to call the CMH Referral Line at (315) 824-6620.
Community Memorial offers primary care in five other locations besides Sherrill (Hamilton,Cazenovia, Morrisville, Munnsville, and Waterville) and warmly welcomes patients for all services, including specialty care.
Community Memorial Expands Orthopedics Team
The Community Memorial Hospital (CMH) network continues to meet the growing demand for specialty patient care with the addition of a new physician assistant to the orthopedics team.
JACKIE IRELAND, PA-C, works alongside Dr. Kamaljeet Banga in both
the outpatient and operating room settings. Ireland joins the CMH team with over a decade of experience, with specialized support in orthopedic spine surgery, holds certifications with the National Commission on Certifications of Physician Assistants, and is professionally affiliated with the American Academy of Physician Associates (AAPA) and the NYS Society of Physician Assistants (NYSSPA).
Graduating from Le Moyne College with a Bachelor of Science in Biology and also a Master’s of Science in Physician Assistant Studies, Ireland has worked in Central New York in a busy orthopedics and spine surgery setting. This experience has lent her the skills, knowledge, and compassion to become a skilled physician assistant and now a member of the Center for Orthopedics at Community Memorial.
With a philosophy of care centering on the patient as a whole person, Ireland remarks, “I chose to join the Community Memorial network because of the close feel of the hospital and orthopedics group. I look forward to providing collaborative care with a practice that offers a variety of sub-specialties and superior orthopedic care.”
Ireland comes to the CMH family as a new location for the orthopedic practice was just opened. Situated on the hospital campus as the new Center for Specialty Services building at 160 Broad Street, the Center for Orthopedic Services moved here in early 2024 to fit the need for expanded patient care space along with a growing team of providers.
Recognizing the important need for local orthopedic care, CMH has dedicated considerable efforts to bolster its provider team and support staff. This concerted activity has led to an increase in volumes, necessitating the expansion of the clinical facility. The Center for Orthopedic Services showcases the expertise of the growing orthopedics team.
“Our medical staff welcomes Jackie Ireland as an esteemed member of our team, bringing expertise and a passion for orthopedic care,” states Michael Walsh, DO, Chief Medical Officer. “She provides a great skill set in education and knowledge with the goal of collaborative community-based care.”
With the Hospital located next door to the orthopedic practice, services are streamlined for patient efficiency. The Center for Orthopedics also offers a walk-in clinic open Monday through Friday, 8am – 1pm, and grants quick appointments following an Emergency Department visit. Patients and providers looking to make an appointment with the Center for Orthopedics at Community Memorial are encouraged to call (315) 824-7040.
Community Memorial Holds Celebration Event
The Community Memorial Foundation hosted a formal celebration event Friday, June 14, 2024, in Hamilton to introduce the second phase of the Hospital’s physical transformation project.
The celebratory affair was held at the Hall of Presidents on the Colgate University campus and brought together organization leaders, medical staff, community members and supporters to highlight the accomplishment of the first phase of the hospital’s transformation project and share what’s to come for the organization with construction now resuming. Phase II will include an expanded and relocated Radiology Department featuring a dedicated Women’s Imaging Suite, a state-of-the-art Laboratory Department, an expanded Rehabilitation Department, and a brand-new Main Entrance for patients and visitors alike.

supporters of Community Memorial and host this event to celebrate
our success and showcase the promising future of the hospital,”
remarked Jeff Coakley, President and CEO. “A night of celebration for our hospital brought together community members in a fun and formal setting. Welcoming guests and supporters to celebrate our success was a great time.”
With dinner and dancing to music provided by the band Grand Central Station, the evening offered an exciting opportunity for attendees to socialize and gain deeper insights into the upcoming physical transformation of the hospital. “Celebrating the success of the completed inpatient wing and all-new Emergency Department while bringing our community members and supporters together is our collective initiative,” added Coakley. “Campaign Co-Chairs Mary Brundage and Dr. Bob Delorme welcomed guests to the evening of celebration, and we appreciate everyone attending.”

A Letter to the Community
2023 was another transformative year for the entire Community Memorial healthcare system, with milestones that reflect the commitment of our Medical Staff, Board of Directors, and all Employees.
Our orthopedic service line has grown quickly to three surgeons and two physician assistants, which you will encounter in this issue. We launched a new professional practice called Hamilton Medical Services (HMS), which allows CMH to expand specialty services more quickly. This new entity is home to specialty services such as orthopedics and the gynecology practice of Scott Beattie, MD, who joined our network in January of 2024.
The Emergency Department of Community Memorial continued to see a record volume of patients, and while we delivered exceptional care, we ensured that patient wait times were 25% lower than the fastest hospital in the region. Primary care services and urgent care have also expanded to meet the growing community needs.
The strategic plan for the organization was finalized in 2023 after extensive work and collaborative efforts from over 200 participants in our planning meetings. Providers, employees and community members all voiced their feedback, perceptions and hopes for the organization. The Admin team established 1-3 year goals to establish a roadmap to the success of Community Memorial.
We hosted a variety of events in 2023 in the communities we serve to provide a platform to receive organizational updates, allow for community input and address questions and concerns. We look forward to hosting more of these in 2024.

- Cazenovia Communities Event – The Hospital partnered with the Cazenovia Chamber for an event to connect communities and shine a spotlight on services and providers at CMH.
- Hosted Several Flu PODS – The Family Health Centers hosted flu PODS at offices for convenient care for community members.
- Rapid Swing Bed Program Growth – The Case Management Department, led by Kelly Coates, RN, CCM, grew the Swing Bed Program to unprecedented volumes, celebrating the short term rehabilitation offered at CMH.

- Hired Senior Director of Business Development – Carrie Bova joined the organization as Senior Director of Business Development, leading the charge on revenue cycle review and reimbursement rates.
- Added (2) NPs to the Cardiology Team – Nurse Practitioners Shelby Sharp and Sherri Loucks joined Dr. Avneet Singh at the Hamilton Heart Center.
- Expanded Ortho team – Frank Walczak, PA, joined Dr. Richelle Takemoto at CMH Orthopedics. Dr. Kamal Banga joined the Orthopedics Program, specializing in upper extremities and sports medicine.
- Expanded Providers to Primary Care – Isaiah J. Hammonds, MD, joined the Waterville Family Health Center as an internal medicine provider for primary care. Kelly North, PA, joined both the Primary Care and Urgent Care teams. Michele Brennan-Vuocolo, FNP, was a welcomed addition to the Morrisville Family Health Center Primary Care Team.
- Opened Orthopedic Walk-In Clinic – The Orthopedic Department launched its weekly walk-in orthopedic clinic.
- General Surgeon Added Service – General Surgeon Dr. Gregory Ripich began a vein ablation procedure and welcomed candidates for consults in both the Cazenovia and Hamilton offices
- Began Construction on the Center for Specialty Services Building – The former Mid-York Building went under construction to become the new location of Hamilton Medical Services, PLLC.
- Announced Finalized Strategic Plan – CMH Administration Team worked with the Rural Health Redesign Center, a federally funded organization, to establish a roadmap for the next three years to lead to a successful future.
- Partnered with CNY Brain and Spine – Dr. Nicholas Qandah of the esteemed CNY Brain and Spine began seeing patients of Community Memorial for neurological care.

2023 was a year of continued rebuilding following the COVID pandemic effects on member enrollment.
- More than 80 members now utilize the CWC.
- Hours of operation were extended for member access and convenience.
- Added flexibility, balance and mobility training to the regular activities of cardio and strength.
- Continued annual fundraisers to support revenue.
- Added two new members to the CWC Board of Govenors: Steve Ceglia and Greg Fuller accepted the positions of retiring six-year Board members Bruce Pierce and Jean Chapin.
- Hired a new trainder, Elise Koen, who brings a degree in Exercise Science and has started a yoga program for members, as a fourth part in the total body workout at the CWC.
CMH Auxilary
The CMH Auxiliary has supported the organization since its inception in 1952, with contributions providing a variety of needs. In recent years, donations have funded the purchase of furniture for medical offices, updated televisions for patients, and telemetry equipment for patient care, along with supporting the costs of equipment upgrades, new technology and programmatic purposes supporting employee education and training.
In May of 2023, the group presented the Hospital with a check for $22,000 to support the purchase of a humidity alarm for the operating rooms.
The 66th Holiday Sale hosted by the Community Memorial Hospital (CMH) Auxiliary was held on Saturday, December 2, 2023, and yielded immense success in support of the Hospital and its five Family Health Centers. This year was one of the most profitable sales, raising more than $10,000 to support the needs of the Hospital. Past funds raised have offset the costs of equipment upgrades, new technology and programmatic purposes supporting employee education and training.
Always actively recruiting and welcoming new members, the Auxiliary meets bi-monthly in Hamilton and participates in a few different annual fundraisers in support of the organization. 2023 Golf Tournament
The Community Memorial Foundation hosted the 7th annual Mike Martin Memorial Golf Tournament to benefit Community Memorial Hospital (CMH) on Monday, July 31, 2023. The proceeds from the competition and supporting sponsors again were designated for the capital project that has completed Phase I and excitedly awaits Phase II.
One hundred thirty-two golfers attended the event, hosted at the beautiful and challenging Seven Oaks Golf Course of Colgate University in Hamilton. With early sun and a few morning showers, the players teed off after a brief welcome at noon. The weather cleared, and after the golfers completed their game, an awards dinner was hosted at the Hamilton Inn.
This event was named to remember a long-time and dedicated volunteer, Mike Martin, who was sadly lost to the community in October 2016. Deeply committed to the Hamilton area, Martin served on the board of the Community Memorial Foundation and chaired the tournament in its very successful inaugural year.
The event raised more than $109,000 in its 7th year and was supported by The Hayner Hoyt Corporation as Tournament Sponsor, SCP Health as Presenting, BPAS as Course Sponsor and NBT Bank as Contest Sponsor.

Hamilton Hearts
In her 70s, Erin Dineen was no stranger to hospitals. A liver transplant survivor from 15 years ago, Erin also beat breast cancer. On top of all that, she also had a valve in her heart replaced about seven years ago.
Erin had her regular cardio check-ups with Dr. Avneet Singh at the Hamilton Heart Center of Community Memorial, where it was learned that the valve previously replaced was starting to close, and Dr. Singh advised she prepare for another bypass procedure.
In the summer of 2023, Erin was home with her private nurse who noticed her skin color was concerning, and called 911. Taken by ambulance to the new Emergency Room of Community Memorial in Hamilton with difficulty breathing, Dr. Singh came immediately to meet Erin in the ER, and advised she be transferred to a cardiac catheterization lab for evaluation and treatment. “I don’t remember too much of what happened and in what order,” noted Erin. “The experience was frightening and it felt like things happened so quickly. I do remember Dr. Singh being in the ER and discussing my best treatment plan, which put me at ease.”
Cardiac catheterization labs offer treatment for heart or blood vessel problems, such as clogged arteries. A thin, hollow tube called a catheter is guided through a vessel to the heart and this procedure gives medical providers important details about the heart muscle and valves. Taken to St. Joseph’s in Syracuse, Erin needed a coronary artery bypass graft, a surgery that involves taking a blood vessel from another part of the body and attaching it to the coronary artery above and below the current blockage.

Dear Community Members,
Community Memorial welcomes the New Year and looks forward to continuing to grow high-quality healthcare services in our region. 2023 was a transformative year for our healthcare system. We opened a new orthopedic practice, expanded our primary care team, set a record of Emergency Department visits, and developed a new strategic plan. All these milestones are a testament to the commitment of our board of directors, medical staff, and employees.
To start the new year off strong, we are excited to introduce the Center for Women’s Health Services and welcome Dr. Scott Beattie and Amy Roberson, PA (previously Medical Arts Gynecology and Infertility) to the Community Memorial family in January. Their gynecology and fertility specialties will certainly enhance the network of services available at Community Memorial.
In the development of this new service line, we are using a captive professional corporation, Hamilton Medical Services, PLLC (HMS). This practice allows CMH to expand its services and continue to grow to meet the needs of our community. As you see HMS throughout the region, rest assured that you will continue to receive the highest quality of care that you have come to expect from Community Memorial.
Finally, we are proud of the development of our new strategic plan, a result of collective efforts from over 200 participants in our strategic planning meetings. Providers, employees, and community members all voiced their feedback, perceptions and hopes for Community Memorial. Community Memorial Primary Care Offices Receive National Recognition
The Community Memorial (CMH) network is excited to announce that each of its five Family Health Center locations, offering primary and specialty care services, have been recognized by a national group as patient-centered medical offices, for delivering primary care and managing the full spectrum of healthcare needs.
The New York State Department of Health collaborated with the National Committee for Quality Assurance (NCQA), the creator of the patient-centered medical home (PCMH) program in developing a transformation model that encompasses all eligible primary care providers in New York State. The NYS Patient-Centered Medical Home (PCMH) Program’s goal is to expand access to high-performing primary care, which drives improved health and wellness in the communities served.
The PCMH model of care enables CMH teams to put the focus on the patient, creating a symbiotic relationship across the care continuum. The providers and staff in the family health centers strive to provide every patient with quality care and a great patient experience. “This accreditation recognizes the dedicated efforts of the Family Health Centers team and their commitment to providing quality improvement with a patient-centered approach to care,” stated Maria Grice, Vice President of Physician Services. According to the NCQA website, the PCMH model ‘…focuses on care coordination, population health, evidence-based guidelines and effective use of health information technology to meet patients’ needs’. The CMH Family Health Centers worked to improve the patient experience, deliver improved preventative care, and collaborate with other clinicians for patient care to identify care gaps.
All primary care practices that have earned recognition through NCQA have made a commitment to improving primary care by promoting the Triple Aim: better health, lower costs and better patient experience.

After about an entire month at St. Joe’s for her heart issue, Erin went to a short-term rehabilitation center for another three months for her recovery. It wasn’t until late fall 2023 that Erin was able to return home, and then to the Center for Cardiology at Community Memorial for outpatient cardiac care.
A common post-op complication developed, atrial fibrillation, or AFib for short, which is typically monitored and cared for with medication. An abnormal heartbeat, AFib is caused by extremely fast and irregular beats from the upper chambers of the heart. With regular check-in visits with Dr. Singh, Erin is happy to be on the mend and return to her everyday activities.
Working alongside Dr. Singh are two nurse practitioners, Shelby Sharp and Sherry Loucks, who both contribute to the collaborative care provided to those needing cardiac services. Bringing over two decades of expertise in both inpatient and outpatient cardiology care, Sherri Loucks has also garnered valuable experience in hospital medicine during her tenure as a Hospitalist in the Utica region. Loucks holds an impressive academic background, earning her Masters of Science in Nursing from SUNY Health Science Center in Syracuse, NY. Furthermore, she obtained her Family Nurse Practitioner certification from Community-General Hospital in Syracuse and graduated with a Bachelor of Science in Nursing from Utica College of Syracuse University.
Shelby Sharp received her Bachelor of Science in Nursing and Master of Science in Nursing with Family Nurse Practitioner from LeMoyne College. Sharp comes from a background of work in cardiology settings, and has worked within the Community Memorial primary care team since 2019. She continues to support her patient panel there, but offers additional and valuable skillsets to the Cardiology Department.
Together, the team at the Hamilton Heart Center offers outpatient care for adults including pre-operative consults and care following heart attack, surgery and other heart conditions. With diagnostic testing and treatments provided right in Hamilton, the Cardiology Department works with patients and their primary care provider to meet every facet of cardiac care needs.

Orthopedic Practice Expansion
CMH welcomed three new practitioners to its expanding orthopedic practice, collaborating for patient care as team members with Dr. Richelle Takemoto, who joined the organization in early 2023.
DR. KAMALJEET BANGA, MD, FRCSC, ABOS, FAAOS Joined The Practice As A Specialist In Orthopedics And Sports Medicine. Dr. Banga’s career has been marked by a strong background in sports medicine, upper extremity, military, and trauma care in both university and community healthcare settings.
“I am truly excited to become a part of the Community Memorial team. It is my privilege to join such a dedicated group of healthcare professionals who share a common goal of delivering exceptional orthopedic care,” stated Dr. Banga. “I am eager to bring my expertise to benefit our patients and contribute to the continued growth and success of Community Memorial’s Orthopedic Center.”
Dr. Banga is certified by the medical boards in the United States, Canada, and India. He holds board certification in sports medicine and orthopedics from the respective medical associations of these nations. Dr. Banga completed his fellowship in Orthopedic Surgery, Upper-Extremity, and Sports Medicine at McMaster University in Ontario, Canada.

FRANK J. WALCZAK IV, PA-C, ATC, CSCS, joins as a Physician Assistant with a foundation in orthopedics and athletic training and will work alongside Dr. Richelle Takemoto. Walczak received a Master of Science in Physician Assistant Studies from Le Moyne College in Syracuse, NY, and a Bachelor of Science in Kinesiology with a concentration in Athletic Training from the State University of New York at Cortland. Mr. Walczak holds multiple national certifications, including a NCCPA Certified Physician Associate (PA-C) certification, NSCA Certified Strength and Conditioning Specialist (CSCS), and NATABOC Certified Athletic Trainer (ATC).
DR. NICHOLAS QANDAH, NEUROSURGEON, now brings his expertise to Community Memorial. Board-certified and fellowshiptrained, he’s and a graduate of the University of Michigan, earned his medical degree from Midwestern University and following his Neurosurgery Residency at Virginia Tech Carilion Clinic, he completed the prestigious AO Complex Spine Fellowship in a dual Orthopedics and Neurosurgery program at the University of Washington-Harborview. Dr. Qandah commenced his postresidency career as an Attending Neurosurgeon and Professor of Neurosurgery at Virginia Tech Carilion Clinic, where he held roles of Director of the Complex Spine Neurosurgery Program and Assistant Residency Director.

With over 15 years of Neurosurgical experience, Dr. Qandah’s comprehensive approach covers various neurosurgical needs, including cervical and lumbar spine care, spinal injuries, scoliosis treatment, and brain trauma. In 2018, Dr. Qandah’s significant contributions led to the establishment of CNY Brain and Spine Neurosurgery in New Hartford, NY, a practice that has flourished under his guidance.
With the new team, CMH required additional patient care space to accommodate the growing practice. With this, a state-of-the-art orthopedic practice is slated to open on the hospital campus in the former Mid-York Building, which is scheduled for early 2024. This facility will also host other visiting specialists, as the Community Memorial Leadership Team actively recruits other essential providers to meet the diverse needs of patients.

General Surgeon Adds Venous Ablation
Dr. Gregory Ripich, Community Memorial General Surgeon, has expanded services offered locally through the addition of a new procedure for varicose veins.
Vein ablation, a treatment to close off varicose veins from within the vein, is a minimally invasive outpatient procedure. Varicose veins can cause pain, heaviness, and swelling and amongst other symptoms, can be cosmetically unappealing to patients.
Diseased veins allow blood to pool, leaving visible varicose veins. Dr. Ripich inserts a catheter into a diseased vein to close it, which re-routes blood to other healthy veins. Affecting more than 30 million people in the United States, varicose veins are twisted and enlarged veins most commonly in the legs, and Dr. Ripich and the team at Community Memorial are excited to offer this relief treatment locally.
Gregory Ripich, MD, is a general surgeon with board certification from the American Board of Surgery. Dr. Ripich attended SUNY Health-Science Center in Syracuse, NY, and completed his residency and training at St. Joseph Mercy Hospital in Ann Arbor, MI. With office visits in both the Cazenovia and Hamilton Family Health Centers of Community Memorial, Dr. Ripich is welcoming patients for both medically necessary and cosmetic cases.

Community Memorial Expands Women’s Health Services with Addition of Gynecology Practice
THE COMMUNITY MEMORIAL (CMH) NETWORK IS THRILLED TO ANNOUNCE its continued growth through the acquisition of a well-established private gynecology practice in Utica, NY.
Dr. Scott Beattie and his team from Medical Arts Gynecology and Infertility, P.C. have joined forces with the Community Memorial healthcare network, solidifying their commitment to providing vital women’s health services in the existing office on Old Burrstone Road in Utica and now also in Hamilton. The longtime dedicated practice will transition to Hamilton Medical Services Center for Women’s Health.
Board-certified by the American Board of Obstetrics and Gynecology, Dr. Beattie graduated with a dual Bachelor of Arts in Biology and Chemistry from SUNY Oswego. He pursued his medical degree at SUNY Upstate Medical University in Syracuse and completed his residency in obstetrics and gynecology at the University of Vermont Medical Center. Beattie specializes in minimally invasive and advanced laparoscopic surgery, offering a wide range of gynecological services for women.

Exceptional Orthopedic Care Made More Accessible
Community Memorial (CMH) in Hamilton continues to expand the orthopedic services provided to the community, offering a new service; a walk-in orthopedic clinic. Dr. Richelle Takemoto will welcome walk-ins each Tuesday and Thursday from 9am – 1pm at her office on the CMH campus effective November 6th.
Dr. Takemoto is a board-certified orthopedic surgeon who completed fellowship training in Orthopedic Trauma. She specializes in comprehensive fracture care, post-traumatic reconstruction, and total joint arthroplasty of the shoulder, knee and hip. In February 2022, after completing extensive training and certification, Dr. Takemoto became the first female surgeon in the world to use the VELYS™ Robotic-Assisted Solution and developed a successful robotic surgery program at CMH. Dr. Takemoto will continue to expand this service as part of the CMH team.
“With no appointment required, patients are able to take advantage of the accessible but vital orthopedic care offered right at Community Memorial,” noted Dr. Michael Walsh, Chief Medical Officer. “Our community and surrounding areas will benefit from the immediate care for sprains, minor fractures and sports-related injuries, with streamlined access to medical imaging in the hospital and follow-up appointments with our Orthopedic program.”
The walk-in clinic will treat patients of all ages as Dr. Takemoto is experienced in the care of children and adults. Staff will assist in coordinating follow-up care at the time of discharge for those patients that may need it. Any patient with a life-threatening injury such as open fracture, excessive bleeding, head or spine injuries should utilize an Emergency Room for treatment.

Take a look at some of our frequently asked questions!
1. If I am picked up by ambulance for transport, can I choose which hospital to go to?
EMS personnel are trained to evaluate care needs for patients and can advise on the facility in which patients may need to go to – and, the level of care provided by different facilities is a factor, also. That being said, patients do have the right to request Community Memorial as their hospital of choice and they can let their EMS transport team know this is their preference.
2. If I visit the Emergency Room of Community Memorial, will my primary care provider know?
Yes. When you register as a patient and let our team know who your primary provider is, we are able to securely send your visit information in an effort to streamline care both in the ER and for follow-up. RHIO (Regional Health Information Organization) HealtheConnections includes more than 10,000 physicians in New York State and supports the dedicated health information exchange between healthcare providers.
3. My doctor ordered lab work / medical imaging, can I choose where I go for care?
Yes! Both our Radiology and Laboratory Departments offers outpatient services to patients, with some not even requiring an appointment. Whether the ordering medical provider is part of our CMH medical team or not, we welcome any and all patients. Primary care providers, urgent care locations, specialty care providers, including surgeons, either provide patients with a hardcopy ordering requisition, or can electronically send orders. We have the capability to send imaging results to any medical professional, so we invite patients to utilize the services provided right here in Hamilton. Often, patients may get a more convenient appointment time for their testing when choosing Community Memorial. To make an appointment with the Radiology Department of CMH, please call (315) 824-6180. To let your medical provider know you would like your testing done right in Hamilton, ask them to send the order to fax number (315) 824-6534. Our Laboratory Department blood draws are offered Mon through Fri, 8am-6:30pm but accommodations may be made for additional appointments. 4. I’m having trouble accessing my medical chart (patient portal) online. Who do I contact or call?
Patients that visit the Hospital for service will have a MyHealthChart account, and patients that utilize our Family Health Centers will have a separate patient portal referred to as ‘Health Center Records’. Both programs offer vital visit and treatment information and we encourage patients to utilize both. Our dedicated staff in both our Health Information Department (HIM) or the Family Health Center Team can assist patients with patient portal issues, including access, navigation and password reset needs. To contact the Medical Records Department directly for questions and assistance, call (315) 824-6580.
5. I’m coming to Community Memorial for surgery. Can my family come with me or visit me?
With COVID restrictions lifted, Community Memorial is excited to welcome back visitors for our patients. Please note the following visitation availability for specific departments, to accommodate our patients and their recovery: Medical/Surgical and Special Care Departments: Open visitation is available from 7am-9pm, Monday through Friday. No one under age 12 unless approved by the charge nurse on duty. Operating Room/Recovery Room: No visitors in these areas. Emergency Department: 2 visitors per patient at a time, all other family and visitors are to remain in the waiting room.
6. I have a question about my bill, who do I contact?
Our Hospital Billing Department (along with the Medical Records Department) have relocated to 2498 Rt. 12B Hamilton, across from the new Stewart’s and Den Kelly Auto. Questions about a hospital bill may be referred in-person to the above address or by calling (315) 701.5627. Family Health Center bill questions should go to 315.937.3015. Both may be paid online as well, and details are included on your individual bill. To pay on our website under the ‘ABOUT’ section: CommunityMemorial.org. 7. Who can I contact regarding my experience at Community Memorial? Larry Baker Family Library
Driven by personal motivation, Larry Baker established the Larry Baker Endowment Fund for Behavioral Health in 2014 and in 2019, the first project funded by the endowment was instituted; Mr. Baker is proud to now offer the Larry Baker Family Library within the Hamilton Family Health Center of Community Memorial. Located in the waiting room, there are books available for both adults and children related to mental health topics. These resources are tools for parents to learn more about experience, struggles, or ongoing behavioral and mental health issues their children may be facing.
“Each year, with the interest earned from the Larry Baker Endowed Fund for Behavioral Health, we’re able to purchase books to re-stock the Larry Baker Family Library shelves,” noted Maria Grice, Vice President of Primary Care. “These resources have been so useful for our patients that we find the books are depleted rather quickly, leaving the shelves empty.”
In an effort to keep the shelves stocked for more to utilize the resources and their information provided, Community Memorial is opening the opportunity for donations of both new and used books related to behavioral health topics. “We’ve had a local retired psychologist donate some of her books for patients and family members to take advantage of and found this is a great way for donors to recycle material they have at home and have it put to good use,” added Grice.
“We hope that the Larry Baker Family Library will encourage conversations about mental health in the context of primary care, and will provide resources for families to use to help their children manage common concerns like anxiety, depressed mood, and hyperactivity,” stated Dr. Jennifer Meyers, Pediatrician at the Hamilton Family Health Center.
Mr. Baker is thrilled the library is available and used to such a great extent, and hopes that it serves as one more support method for local families. “I’m so pleased the books are being put to such good use; now, we’d like to make sure the shelves stay stocked for even more patients and family members to have access to,” said Mr. Baker.

Women’s Health at Community Memorial
The fall season means many things, but one consistent and important part is the annual recognition and awareness month for breast cancer. Each October, healthcare providers work to raise awareness about the impact of breast cancer and also to remind patients to get their annual screening exams.
“Education is the starting point,” stated Dr. Jocelyn Morin, Family Practice Provider of the Community Memorial Family Health Center in Cazenovia. “Once patients understand and are more aware of their personal risk factors along with signs and symptoms, they can stay up to date on preventative screening needs.” According to the American Cancer Society, when detected early and in the localized stage, breast cancer has a 5-year relative survival rate of 99%. Early detection means patients do monthly self-exams of their breasts and schedule regular clinical breast exams and mammograms. “Women of all ages are encouraged to stay up-to-date on their self-exams and talk to their doctor right away if they notice anything abnormal,” added Dr. Morin.
At Community Memorial in Hamilton, a ‘fast-track’ system is in place for mammography patients, offering direct access to a dedicated waiting space after a simple registration. Certified technicians have years of specialty imaging experience, providing patients with compassionate, personalized healthcare. Comprehensive 3D-mammography services are available, with state-of-the-art ultrasound and mammography machines. Same-day sonograms for post-mammography needs are also available in some cases.

Coming Next: The ongoing transformation project will bring a renovated and expanded Radiology Department, relocated next to the new Emergency Department. This larger space will also feature a new Women’s Imaging Suite, designed with the patients in mind, offering a new and private waiting space dedicated to those patients getting a mammogram. A Letter to the Community
Community Memorial marked our 70th anniversary in 2022 and the Board of Directors, Providers and staff remain committed to providing the highest quality of care. Through the final months of the COVID pandemic in 2022, we witnessed the perseverance, dedication, and compassion of the frontline providers and staff of Community Memorial. I quickly learned what sets Community Memorial apart from other hospital systems: our people.
The Providers and staff at CMH worked through the most challenging time in healthcare, caring for the sickest patients who have come through our doors, with the most significant staffing shortages the industry has ever seen.
Despite the COVID pandemic’s challenges, we had many successes in our 70th year. In July, we gathered with Providers and staff to celebrate our legacy of providing healthcare in our community. We welcomed new medical staff, expanded our services and advanced our medical technology as a testament to our commitment to expanding and growing our services.
2022 was a year where we saw challenges as an opportunity to become stronger, more aligned and more focused as an organization. The Board of Directors and Administration team look forward to completing the strategic plan that will let us prioritize our focus areas and align our strategic initiatives. We strive to continue our development of service lines that will meet our community’s needs and ensure that we can continue to deliver the highest quality of care for another 70 years to come.

Avneet Singh, MD, FACC
Cardiology
Community Memorial welcomed Dr. Aveneet Singh in October of 2021. Dr. Singh studied medicine at Maulana Azad Medical College and graduated with his Bachelor of Medicine and Bachelor of Surgery in New Delhi, India. Singh also completed his education in internal medicine before completing his residency and fellowship in cardiology at SUNY Upstate Medical University in Syracuse. Dr. Avneet Singh welcomes new patients at the Hamilton Heart Center, where he focuses on patient-centric cardiovascular disease prevention, non-invasive diagnostic testing, and cardiac critical care.
Richelle Takemoto, MD
Director of Robotic Surgery, Orthopedics
Dr. Richelle Takemoto came to the Hamilton community in the fall of 2021, and she quickly began leading the way in developing a robotic surgery program at Community Memorial. As Director of Robotic Surgery at CMH, Dr. Takemoto was instrumental in the relationship with DePuy Synthes and in bringing the new technology to our operating room. In February of 2022, Dr. Takemoto began robotic-assisted surgeries at CMH. She is the first female worldwide to use the VELYS DePuy Synthes robotic-assisted technology. In 2022, Dr. Takemoto broke another record, being the first surgeon to perform more than 100 surgeries with the VELYS Robot.
Gregory Ripich, MD
General Surgeon
In the fall of 2022, CMH expanded its General Surgery team with the addition of Dr. Gregory Ripich. Ripich joins Dr. Martin Ernits in providing patients with elective procedures and surgeries, emergent cases and collaboration with onsite hospitalists. Ripich specializes in various surgeries such as colon/rectal, breast, thyroid, skin and wound. A board-certified surgeon with the American Board of Surgery, Ripich attended SUNY Health-Science Center in Syracuse, NY and completed his residency and training at St. Joseph Mercy Hospital in Ann Arbor, MI. Dr. Ripich sees outpatients in both the Cazenovia and Hamilton offices and offers procedures right at CMH in Hamilton.

- The Transformation Project’s loan guarantee was approved by USDA to pursue construction financing. The revision of the plans and updated project bidding has commenced.
- The CMH Foundation hosted a Meet the CEO event held at the Colgate Inn to introduce Jeff Coakley to community members.
- Jason Liebel is welcomed as the head of Finance in collaboration with Finance Consultant Jason Lesch.
- The new Leadership Performance Improvement (PI) Committee led by Dr. Walsh and Heather Bernard commenced work on centralizing the quality assurance functions of Community Memorial.
- Jeff Coakley and Dr. Michael Wolk presented the Future of Healthcare for the first time at the Colgate Alumni series.
QUARTER 3
- The Board of Directors made a motion to accept the revised bylaws of CMH with Crouse Health removing Crouse control of the Board.
- Dr. Trevor McGinley and P.A. Amy Rowe were named Co-Directors of the Emergency Department.
- CEO and staff participated in the Hamilton Independence Day Parade.

CMH Auxiliary
The Auxiliary of Community Memorial was eager to resume its meetings after a long hiatus due to the pandemic. After two years, the group began its regular meetings in March of 2022.
On March 16, 2022, the Auxiliary presented the hospital with a check for $20,000. The check represented the final payment of the committee’s pledge of $100,000 to the hospital to support its capital transformation project.
The Auxiliary’s Holiday Sale also made a return in 2022. After months of planning and volunteer committee, the group held the event at St. Thomas’ Church in Hamilton on December 3, 2022, and raised $8,000.
The Auxiliary continues to fundraise and support the hospital through volunteer hours. Volunteers assist our main entrance staff during visiting hours, completing visitor registration.

2022 Golf Tournament
The Community Memorial Foundation hosted the 6th annual Mike Martin Memorial Golf Tournament to benefit Community Memorial Hospital (CMH) on Monday, August 1, 2022. With ‘Putting for Progress’ as the theme, the proceeds from the competition and supporting sponsors went towards the capital project currently underway at Community Memorial Hospital, as the organization celebrated its 70th anniversary.
One hundred twenty-eight golfers attended the event, hosted at the beautiful and challenging Seven Oaks Golf Course of Colgate University in Hamilton. This event was named to remember a long-time and dedicated volunteer, Mike Martin, who was sadly lost to the community in October 2016. Deeply committed to the Hamilton area, Martin served on the board of the Community Memorial Foundation and chaired the tournament in its very successful inaugural year. The tournament recognizes his leadership skills, commitment to the hospital and the local community, and wonderful personality.
The sun was out all day with typical warm August golf weather, and after a brief welcome at noon, the players tee’d off. After the golfers completed their game, an awards dinner was hosted at the Hamilton Inn. The event raised almost $100,000 in its 6th year and was supported by The Hayner Hoyt Corporation as Tournament Sponsor and SCP Health as Presenting Sponsor – thank you to every one of the event sponsors, participants and volunteers for making this another success!

Changing to Meet The Needs of Today & Tomorrow
In June of 2020, after years of extensive review and planning to address facility, staff and patient needs, Community Memorial began its largest capital project since its inception in 1952. Impacting and changing over half the existing facility, this project took aim at the most pressing patient care areas to bring the physical organization to the same level of superior care provided within its walls.
New Inpatient Rooms
In January of 2021, Community Memorial celebrated the opening of its new inpatient medical/surgical wing. The hospital contracted with Hayner Hoyt
to convert the vacant and former Skilled Nursing Facility wing into fourteen brand-new, private inpatient rooms. After approximately six months
of construction, the wing was opened during the height of another local COVID surge, welcoming patients with added negative-pressure treatment rooms to adequately care for any infectious disease. The new rooms provide a modern atmosphere
and comfortable setting for both patients and their visitors during their hospital stay. The wing includes a centralized nursing station for staff to more efficiently care for patients, with direct view into the rooms of the most critical patients, and is conveniently located adjacent to the Special Care Unit of the Hospital.
Emergency Department
The new Emergency Department (ED) opened in December of 2021 with an expanded waiting room, private patient bays, and enhanced technology and safety features. The expansion includes three additional rooms, bringing the total number to ten and increasing patient privacy and safety. The ED also has enhanced technology, including a centralized telemetry system to monitor patient cardiac status, and security features that restrict entrance to the ED to those with badge access. And now, the department offers its own designated entrance and waiting space, separating the sick and well patients in an additional effort to keep all patients safe.
CMH has added a decontamination room with private access through the outdoor showers.
This will allow patients, EMS providers and staff to rinse any potentially hazardous chemicals before entering the building. The decontamination room has an anteroom to give staff the space to safely don and doff personal protective equipment
and minimize the transfer of any hazardous waste or airborne diseases with an enhanced HVAC system. A decontamination room in a rural community is critical in providing safe care for our patients and staff.
The new Emergency Department opened as our region entered the highest positivity rate for COVID-19 since the onset of the pandemic in March 2020. The added space was put to good use as our staff cared for a historic volume of patients that required more care, and has continued to serve exceptional care since, with 2022 seeing the highest volume of ED patients ever.
What’s Next
With the first phase successfully completed, upcoming in the project timeline is a temporary relocation of the Laboratory Department while the current space is renovated and expanded. Once completed, the new department will provide a waiting space that can accommodate more patients and is also ADA compliant, adding waiting space and doorways that are wheelchair-friendly.
Another large portion of the project will be the relocation of the Radiology Department, including a new CT Scanner and MRI machine, which will offer patients superior technology with enhanced medical imaging. The expanded department space will sit directly next to
the already-completed Emergency Department, ensuring prompt and efficient vital imaging services are provided in the most emergent cases. The new space will also offer a modern and comfortable Women’s Imaging Suite, to align with the women’s health services provided within the Community Memorial network.
Still to come will also be the renovation of the Rehabilitation Department, which offers both physical and occupational therapy for both inpatient and outpatient care. The new spaces will upgrade equipment vital to therapy needs, increasing services provided to patients. And, the main entrance of the organization will shift slightly, to accommodate both the expansion of the new Radiology Department once completed, but also to streamline patient and visitor flow when entering the building.

Cardio Care
In the midst of 2022, Marney Rowland, age 53, was an avid walker, logging anywhere between four to seven miles daily. The tread on her sneakers showed her success, and the scale did, too, as she’d begun losing weight and feeling healthier. Dropping 25 pounds, Marney noticed her heart felt like it was skipping beats.
Community Memorial Primary Care Physician, Dr. Kerri Taylor, adjusted some of Marney’s medications – but the condition worsened. When she began to see some kaleidoscope colors in the corners of her eyes, Dr. Taylor referred her to Dr. Avneet Singh, Cardiologist at Community Memorial. “I had seen a cardiologist about six years ago but hadn’t had any abnormal findings,” noted Marney. “These symptoms and everything that came after was unexpected.”
In early November, Dr. Singh’s office performed an EKG (electrocardiogram) on Marney – a test recording the heart’s electrical signal to check for different heart conditions. After seeing some concerning results, a second EKG was performed immediately following. The next day, the office called Marney and asked her to return for a nuclear stress test, which uses an injection and an imaging machine to create pictures of the blood flow to the heart. Marney was then sent to Crouse Health in Syracuse for a heart catheterization, a procedure in which a thin tube is guided through a blood vessel into the heart.
“The doctor came in and said I had blockages and needed a five-way bypass, and I was sent to Upstate Hospital, next door, right away,” added Marney. “They kept me a few days, and then I had surgery. Seven hours in the OR for what ended up being a quadruple bypass, then five more days as an inpatient, and I went home.”
Post-op, Marney has some fluid still on her lungs, and her care team adjusts her medications and monitors her blood levels weekly. “Dr. Singh advised I will most likely be on A-Fib medication for a couple of months, which is common,” stated Marney. “I feel better than I did yesterday but not as good as I’ll feel tomorrow.”
Home recovering, Marney notes the appreciation for her long-time primary care provider. “I’ve seen Dr. Taylor for 20 years, I love her – and now, I’ve got a great Cardiologist with Dr. Singh. He has a great attitude with patients and always lifts me mentally.” Originally from the Oneida and Sherrill area, Marney moved north a few years ago, now traveling for an hour to see Dr. Taylor and Dr. Singh. “When you have a great medical provider you love, it’s worth going the distance to them.”

Cardiologist
Dr. Singh is a Fellow of the American College of Cardiology and is certified by the American Board of Internal Medicine (ABIM) in Internal Medicine and Cardiovascular Disease. He is also certified by The National Board of Echocardiography (NBE), The Certification Board of Nuclear Cardiology (CBNC), and The Certification Board of Cardiovascular Computed Tomography (CBCCT). He is welcoming new patients at the Hamilton Heart Center.
Rebecca Dorn, FNP-C
Nurse Practitioner, Cardiology
Dorn earned her Associate’s degree in Applied Science in Nursing from Fulton Montgomery Community College and her Bachelor of Science Nursing degree from SUNY Plattsburgh. She also obtained her Master of Science in Nursing as a Family Nurse Practitioner from SUNY Upstate Medical University in Syracuse. Dorn has more than 30 years of nursing experience, most recently specialized in providing urgent care services. She has also provided care to patients as a Hospitalist and previously served as an Emergency Department Director. She is welcoming new patients at the Hamilton Heart Center.
CEO Update
Welcome to the New Year!
2022 was undoubtedly a trying year for our providers and staff as we adjusted to this new pandemic era of healthcare services. This environment created some of the most significant staffing challenges of our time while we courageously served historical volumes of patients in our Emergency Department. These patients from our community also came to us requiring a higher level of care, which challenged our hospital as well as other regional medical centers that we rely on to accept our most critically ill patients when they needed advanced services. Caring for our patients is our number one priority, and we remain committed to increasing services to patients in this community.
In 2023 we brought back our February Go Red cardiac health program with Colgate University. This event featured our Cardiology team; Dr. Avneet Singh and Rebecca Dorn, FNP-C. Also coming up, we will host a free health clinic with General Surgeon Dr. Gregory Ripich for community members to get more information on hernias, prevention and treatment, as well as a free hernia screening. It is also important to CMH that we provide critical access to primary care through urgent care and same-day services in our five Family Health Centers.
Our ongoing transformation project will resume this year as well. Construction kicked off in 2019, with plans to modernize over half of our facility. While we completed the construction of our new Inpatient wing and Emergency Department, we were ultimately caught in the downstream impact of the global pandemic. Labor and supply costs have challenged the project, and the economy has continued to drive up financing costs. Community Memorial considered all options for moving this project forward. After months of consulting and consideration, we are excited to look forward to making good on our promise to the community to transform our facility and medical equipment to match the quality care that our team provides.
I want to acknowledge the hard work and dedication of our Community Memorial Auxiliary, a team of volunteers who have supported our team in many ways. Heavily impacted by the pandemic, their ability to host their fundraisers and perform their cherished volunteer activities in the hospital was drastically reduced. Thus, they persevered and continued to do everything they could to support CMH, and they had an incredible turnout at 2022’s December Holiday Sale. In this edition, we share information on how to get involved with this group if you wish to learn more.
Seasonal Health: COVID vs. FLU vs. RSV
Regionally, hospital Emergency Departments, Urgent Care offices and primary care providers have experienced high volumes of illness in recent months, specifically respiratory illnesses. In the height of the pandemic, social distancing, mask mandates and increased hand washing practices impacted and decreased rates of flu and RSV – but since restrictions have been relaxed, rates of illnesses have spiked, with associated hospitalizations following.
COVID – CORONAVIRUS
COVID-19 is caused by SARS-CoV-2, a new coronavirus. Coronaviruses are a family of viruses named for their corona-like shape. Sometimes, the general term “coronavirus” is used with COVID-19, but this is technically incorrect because there are many types of coronaviruses in this family. Vaccines and booster shots are available to combat this virus.
FLU – INFLUENZA
The flu is caused by the influenza virus and spreads quickly during the winter months when people spend increased time together indoors. There are many influenza virus strains, which can change yearly, so you should get a flu vaccine each year.
RSV – RESPIRATORY SYNCYTIAL VIRUS
RSV is caused by the respiratory syncytial virus, a highly contagious virus that can infect children and adults. In adults and older children, RSV is typically a mild illness similar to the common cold. In infants and the elderly, the symptoms can be more severe.
CMH Wellness Center
The Community Wellness Center of CMH offers guests and members the important benefits of exercise while encouraging healthy practices in individual nutrition goals. And, the personal trainers on staff can educate on strength training at no additional cost.
Personal exercises for individuals and groups are designed and monitored by the trainer and paired with guidance to ensure the safest and most beneficial use of fitness center time. Open to anyone looking to get healthier, the Wellness Center combines important topics of fitness, nutrition and education to make the biggest impact on maintaining health and wellness for the body and mind.
With unlimited use of the facility, there’s an excellent selection of professional equipment with instructions on the safest and most efficient way to use everything from the elliptical-cross trainers to treadmills and weights. The friendly and social environment encourages members and guests of all ages and abilities to feel supported, no matter their exercise routine preference or experience level.

The Wellness Center emphasizes a total body workout, targeting activities that feature balance, mobility, strength maintenance, cardiovascular health and more. Also available is physician input for programs to be tailored to goals and ability levels if participants prefer. As a community outreach for Community Memorial Hospital, the highest standards for a healthy and clean environment are met, with strict cleaning practices to ensure the safety of all.
Affordable monthly membership fees mean patrons do not have to travel to join a gym, and a local wellness program is instrumental in the community. Regular exercise has many benefits beyond physical, including the positive impact
on reducing mental health disorders such as anxiety and depression while improving self-esteem. Exercise has been found to alleviate social withdrawal. The promotion of psychological well-being is continually fostered, with the consistent messaging of ‘come, try and succeed’!
Located on Route 12B in Hamilton next to S&S Appliances, this space offers adequate parking and a comfortable atmosphere for workouts. Current hours of operation are 6 am-noon and
1 pm-5 pm, Monday through Friday. Multiple membership
plans are available, including monthly and quarterly rates for individuals, couples and families (guests must be 14 or older and be accompanied by an adult).
The Wellness Center has served the community for over two decades, and the team invites those interested to come visit to observe and learn more about the facility, program and more!

Welcoming New Members!
The Auxiliary of Community Memorial (CMH), founded even before the hospital opened its doors, was formed to support the new organization in various
ways. Each year, several fundraisers are held, with all proceeds benefiting the Community Memorial health network. Funds raised have provided the hospital with vital equipment, furniture for physician offices, televisions for patient rooms and more.
As volunteers, the Auxiliary members aid the hospital in other ways, including supporting the reception desk, assisting with patient navigation, and patient and visitor temperature screenings. Additionally, the Auxiliary hosts the popular Holiday Sale held in the community each December.
The Auxiliary presents an opportunity to engage with and meet new people, try out skills and uncover hidden talents to make valuable personal contributions. There is always room for new members, and the Auxiliary welcomes anyone interested in volunteering to attend a meeting to learn more or reach out to the Auxiliary President, Gloria Gaudreau, at the information listed below. How much one volunteers is up to the individual – a few hours a day, week, or month, or on special projects; giving of oneself has many rewards and personal satisfaction. There are no restrictions on membership – all ages are welcome. Currently, there are bi-monthly meetings held on the hospital campus in Hamilton.

Community Memorial Welcomes Dr. Gregory Ripich, MD
The Community Memorial (CMH) healthcare system welcomed Gregory Ripich, MD, who joined the organization’s surgery team as a General Surgeon in August of this year.
AREAS OF SPECIALITY INCLUDE:
• Advanced minimally invasive surgery
• EGD – Upper Endoscopy
• Colon and rectal surgery, including colonoscopy screenings
Dr. Ripich joins Dr. Martin Ernits in the General Surgery Program at CMH; this addition provides the community with a greater scope of care and allows CMH to care for a greater volume of patients. Ripich will perform elective procedures and surgeries, specializing in various surgeries such as colon/rectal, breast, thyroid, skin and wound. Dr. Ernits will continue to perform emergent surgeries and collaborate with onsite hospitalists.
Ripich has started seeing patients in the CMH Family Health Centers of Hamilton and Cazenovia. “The Community Memorial team is excited to bring Dr. Ripich on board. He brings a broad range of skills and experience to our patients,” commented Dr. Michael Walsh, Chief Medical Officer. “With his focus on a specialty covering various medical issues, we’re confident that patients will be pleased with the care offered here in Hamilton.”
Ripich performed his postgraduate training at St. Joseph Mercy Hospital in Michigan after receiving his Doctorate in Medicine from SUNY Health Science Center at Syracuse. Prior, he studied at the University of Buffalo, receiving his Master of Science degree. Ripich attended Cornell University for his undergraduate degree, obtaining his Bachelor’s degree in Neurobiology. His extensive career has defined his surgical interest and experience in advanced minimally invasive surgery and endoscopic procedures. He most recently served as the Medical Director for Surgical Associates of CMA in Ithaca, NY.
“Dr. Ripich brings experience and a wealth of knowledge from larger health systems that will allow patients to receive care locally,” noted Jeff Coakley, President and CEO. “CMH is excited for the future of General Surgery in Hamilton. CEO Update
As we prepare to close another busy year, I’d like to take a moment to thank patients that choose Community Memorial for their healthcare needs.
2022 has been a challenging time for healthcare providers across the state and our nation. Following the pandemic, we have found greater healthcare needs in our community and have fewer providers, nurses and healthcare professionals to support the needs. While wait times have grown in Hamilton, they are a fraction of the time patients are waiting in other Emergency Departments across our region.
Community Memorial has an average wait time of 27 minutes for evaluation by a nurse and approximately 43 minutes to see a provider in the past six months. This is significantly less when considering the average wait time in New York State Emergency Departments is 3 hours, according to the Centers for Medicare and Medicaid (CMS). We are proud to be your healthcare provider of choice and ask that you continue to be patient with us as we manage through these challenging times.

Community Memorial Reinstates Graduate Nursing Program
Graduate-level nurses can now take advantage of a residency program offered at Community Memorial (CMH), providing real-time orientation and clinical training in a nursing career, before taking their New York State nursing board exam.
Following the pandemic, the shortage of healthcare workers became a nationwide crisis. This program aims to recruit
and retain nurses to combat the high nurse turnover that has affected the healthcare industry. “National rates of nursing turnover are the highest they have ever been. Reasons for the turnover rates include emotional/physical exhaustion, fatigue, and burnout. The Graduate Nursing Residency Program provides the new graduate nurses a controlled onboarding and orientation that includes classroom and direct supervision of a Registered Nurse that better prepares them for the current workforce by building skills through competency training and a small patient assignment,” remarked Heather Bernard, Vice President of Nursing and Clinical Services.
Until the participants are able to sit for their board exam, they are paid CMH employees and go through a week-long classroom training of policies and procedures and equipment before hitting the inpatient floor for interactive patient care.
The classroom portion also features different focus areas, including specialty classes with presenters, such as wound care and pressure ulcer prevention. Initial program attendees were existing CMH staff, working as they attended nursing school. However, future program openings will be filled as available, through direct interest and requests and local career fair participation.
The success of the program will be monitored through collection of retention rates currently measured by the Human Resources Department of CMH.
“Our goal with this program is to really lay the groundwork for future nurses, to prepare them in patient care, as to avoid any overwhelming feelings when they begin working as a nurse,” noted Christine Martin, RN, Program Instructor. “Nurses work closely with supervision, and are allowed to care for patients and receive imperative training all at once.”
This program was previously paused at CMH over a decade ago, as there was not such a pressing need to fill frontline staff positions. “I’m excited for Community Memorial, and for the nursing graduates to have this opportunity, and I look forward to this new endeavor,” added Kelly Kahler, Hospital Education Coordinator.

Team Care Medicine Launched At Community Memorial Hospital
After months of review, preparation and planning, Community Memorial (CMH) launched a new patient care approach within some primary care offices, implementing a Team Based Medicine Model. Beginning mid-October, this program started with a core group of providers and hopes to expand to all of primary care throughout the CMH network, once levels of success are measured.
The overall goal of team based care is a start-to-finish comprehensive healthcare model, offering patients support and collaboration, along with full participation, in their care needs. Working with multiple healthcare providers and office staff, the model aims to coordinate efforts across all settings of care required for each individual patient.
Teams within primary care settings have demonstrated value with regards to both patient and provider outcomes. “Our main goal is always our patient care and experience; this model will provide that but also improve clinician well-being,” noted Dr. Michael Walsh, Chief Medical Officer. “This care delivery model will also allow better access to your healthcare provider, more focused attention on the patient, and an improved level of care provided.”
Currently offered in Hamilton, Morrisville, Munnsville and Cazenovia office locations and spearheaded by three physicians and two nurse practitioners, this program centers around key players treating a patient, requiring they work together to drive optimal care outcomes. Engaging patients in their care and health outcomes while encouraging providers to utilize all aspects
of their education and experience is a key component of this team approach. At CMH, a nurse or office assistant will remain in the room with the patient throughout their visit, collecting information before the provider enters, documenting while the provider focuses on the patient, and remaining in the room after the provider leaves to provide care instructions, education and answer any questions.
“The implementation of this new program will ultimately have a positive impact on our patients,” stated Dr. Sunny Nelson, Primary Care Provider and program champion. “Through effective communication and collaborative care, clear and common goals will lead to better patient experiences – and allows for more accurate office visits.”
Extensive training was attending by all staff participating in the program in preparation for launch. Success of this program will be measured through a combination of review of quality of care, patient satisfaction, provider and office staff satisfaction, and increased patient access to care.

CMH Cardio Team Expands For Patient Care
The Community Memorial Health Network (CMH) continues to expand, adding a Nurse Practitioner to its Cardiology Practice in Hamilton.
Rebecca Dorn, FNP-C, recently joined the Cardiology Department with Avneet Singh, MD, at the Hamilton Family Health Center of CMH, located on the hospital campus.
Dorn earned her Associate’s degree in Applied Science in Nursing from Fulton Montgomery Community College and her Bachelor of Science Nursing degree from SUNY Plattsburgh. She also obtained her Master of Science in Nursing as a Family Nurse Practitioner from SUNY Upstate Medical University in Syracuse. Dorn came with more than 30 years of nursing experience, most recently specialized in providing urgent care services. She has also provided care to patients as a Hospitalist and previously served as an Emergency Department Director.
“I’m excited about Rebecca joining Hamilton Heart Center. She brings a vast breadth of experience with her as a med-evac nurse and a seasoned nurse practitioner,” remarked Dr. Singh. “I’m certain her addition to our cardiology team will help us deliver prompt, high quality and modern cardiology care to our community.”
“Cardiovascular care at Community Memorial has grown exponentially, and the addition of Rebecca will support even more growth in Cardiology,” noted Jeff Coakley, President & CEO. “Community Memorial is committed to ensuring patients receive access to cardiology services close to home and adding a Nurse Practitioner to Dr. Singh’s practice will make this possible.”
“The opportunity to join a team with plans to grow their cardiology program was very intriguing to me. The growth of
the program, in turn, offers growth in knowledge. What better combination could there be,” remarked Dorn. “The ultimate success lies in the awesome people who make up the team here in Hamilton.”
Community Memorial, in conjunction with Colgate University and Madison County’s Liberty Resources/Help Restore Hope Center, launched the Sexual Assault Program in 2017. The SANE (Sexual Assault Nurse Examiners) program provides specialty- trained nurses, who offer all residents of Southern Madison County comprehensive medical and psychological services specifically related to sexual assault. Additionally, victims of domestic violence that may need forensic evidence collection or psychological care are assisted. Prior to 2017, survivors of sexual assault were required to travel 30 to 60 miles from Hamilton to access SANE services.
In its over five years existing in Hamilton, the SANE program has been supported by the CMH Auxiliary, who donated funds to support the Emergency Room Department in purchasing clothing items for patients that were victims of sexual assault. “When a patient comes to our facility for sexual assault treatment, we often have to collect their clothing as evidence, for forensics. With the funds donated from the Auxiliary, we were able to purchase clothing to have available for patients to wear home in those situations,” stated ER Director of Nursing Bernie Casscles. “This option can make a world of difference in such a sensitive time, and we’re so fortunate to have such a supportive Auxiliary.”
Additionally, when neighboring hospital Oneida Healthcare implemented the SANE program within their facility in 2019, CMH was a welcoming partner as Oneida came onboard, offering even more care options in Madison County for sexual assault victims. The renovated and expanded Emergency Department of CMH, which opened its doors almost a year ago to patients, Community Memorial is able to offer more private treatment spaces, a private shower within the department, more room to maneuver equipment, and increased space for private consultation amongst the SANE nurse, advocate and law enforcement.
SANE services include STD/STI testing, advocate support, counseling, follow up care and forensic medical examinations that collect and secure evidence in the eventuality a survivor chooses to do so and in order to prosecute the perpetrator. Treatment and recovery from such a traumatic event, along with SANE advocates offering longtime care, is extremely vital for any patient that needs it. The SANE program recently expanded their services in August of 2022 to include response assist with medical/psychosocial/ and forensic care of victims of domestic violence, intimate partner violence, non- fatal strangulation, human trafficking, and neglect.

Community Memorial Hospital continued its popular community event on September 16th, with the 4th annual Drive-Flu Clinic. This year, over 270 flu shots were provided for area residents.
Offered in the south parking lot of the organization’s main campus, staff was prepped and ready to go for the first session at 7am on a blustery day. Patients ages 18 and over were able to pull in, fill out a simple form, and get their annual flu shot – all while remaining in their vehicle. This safe, convenient and easy access to a flu shot has continued to be a success, and provided a seamless process for patients. “Community Memorial has offered a free flu clinic for years for our area residents, as a resource to combat seasonal flu outbreaks,” stated Shelly Trost, RN, Infection Preventionist at Community Memorial. “We encourage everyone to utilize as many preventative options as possible with their personal healthcare.”
The Centers for Disease Control (CDC) recommends everyone 6 months and older get a flu vaccine each year. Seasonal influenza is associated with large numbers of illnesses, which can impact school attendance, worker absenteeism, and daily productivity.
Patrons were excited to again see the Drive-Flu event, noting how the convenience benefit encourages vaccination. “Each year, our community residents line up bright and early for this event,” added Trost. “And, it’s a bonus that no appointments are necessary, and household members can come together. We’re happy to continue to provide this important service.”

Community Memorial is working through plans to resume our transformation of our facility. Our first two phases converted our previous skilled nursing facility into modern, private inpatient rooms and created a brand new Emergency Department. The remainder
of the project will provide necessary updates to
our medical imaging equipment and centralize our Radiology Department within our facility. The project will also modernize our exterior with a brand new main entrance, and update our laboratory service department.
Like many other organizations, CMH has been met with unique challenges brought on by the economic impact of the pandemic. We have been working with our construction management company, architects, the USDA and lending firm to finalize project details. We’re excited to continue to transform our facility and heighten the patient experience. These new spaces will also allow our organization to continue recruiting talented staff to our team. Community Memorial Begins Robotic Surgery Program
A slippery day in January sent Cindy Eaton down, landing on her forearm, resulting in a bad break of her right arm, between the shoulder and elbow. After visiting a large ER in a neighboring city, a temporary cast was applied but terrible swelling and pain occurred, and Cindy was asked to wait a week to see an orthopedic specialist. She then decided to utilize the relationship between Community Memorial Hospital (CMH) and Community Memorial Center for Orthopedics. Already a primary care patient of CMH, Cindy was aware of the reputation and skill between the two organizations. Offered an immediate appointment, Cindy met the new physician with the group, Dr. Richelle Takemoto, and was scheduled for prompt outpatient surgery the next day at CMH.
“I go out of my way to come to CMH,” states Cindy, a resident of Richfield Springs. “From the moment I walked into the hospital, I was treated with respect and compassion. It was a very traumatic time for me as I was in a lot of pain upon arrival, but the entire team at CMH was wonderful throughout the entire process.”
The stellar reputation of Community Memorial Center for Orthopedics and CMH extends across New York, and Cindy was delighted to capitalize on the care and skill provided by both. Recently, the bar has been raised again as CMH has successfully launched its new Robotic Surgery Program through this partnership with the orthopedic group. With the VELYS™ Robotic-Assisted Solution, Dr. Takemoto performed the first robotic joint surgery with the CMH Surgical Team in mid-February.
After extensive training and certification from DePuy on the VELYS™ Robotic Solution, the innovative equipment arrived, ready to become an extension to the already-superior orthopedic care offered in Hamilton. Community Memorial is the second hospital in New York State to implement this new advanced technology. “The Robotic Surgery Program brings advanced technology into our operating room, providing patients with advanced technology and our same five-star caring service,” states Jeff Coakley, President and CEO. “We are proud to partner with Community Memorial Center for Orthopedics for the Robotic Surgery Program. With thousands of successful knee replacements provided by our skilled surgeons this gives patients one more reason to have their surgery at Community Memorial.”
Dr. Takemoto is excited to expand the program at Community Memorial. As the first female surgeon to use this innovative technology, she understands the benefits of robotics technology to patients. “The robotic technology provides me with greater precision on a cut. I can determine a measurement down the millimeter,” commented Takemoto. “This means increased stability and reduction in pain for patients, providing them with a better range of motion and preventing the unstable feeling some patients experience during everyday activities, such as bending and walking up and down stairs.”

CEO Update
Community Memorial has been providing high quality healthcare for the past 70 years and we continue to invest in our medical staff, clinicians, technology and facility to lead us to an even more successful future.
New medical providers have been welcomed to our team, along with a VP of Nursing. Dr. Kareem Hamad and Dr. Matthew McKay are both Hospitalists and come to CMH from larger facilities with years of experience providing patient care. Eric Engelbrecht and Michael Kiger are both Physician Assistants working in our Emergency Department, and a new Nurse Practitioner, Rebecca Dorn, joined the Cardiology Department. Heather Bernard DNP, RN, CIC, FAPIC has joined our CMH family to oversee nursing and clinical services. I look forward to working together with these new team members.
Our new Med/Surg Unit opened January 2021 and the ER in December 2021, to open and close the year with successful project completions. These updates to our facility also initiated the renovations to the exterior of the building. The dedicated Emergency Department entrance allows walk-in patients to enter the department without using the main entrance. Patients arriving by ambulance with the support of our EMS partners also have state-of-the-art entry to increase access to immediate care.
The Hospital’s Board of Directors and Administration are proud of our Surgical Department’s recent advancement, offering the most innovative program in joint replacement surgeries; robotic technology. The team collaborated with our partners at Community Memorial Center for Orthopedics to bring this to our operating rooms. As the second hospital in New York State to offer patients the VELYS RoboticAssisted technology, our Director of Robotic Surgery, Richelle Takemoto, MD, is the first female surgeon in the United States to use the VELYS robot.
Lastly, I would like to thank our employee team for their support and dedication to patient care and our community. The quality of care provided at CMH has again earned us a five-star rating in patient satisfaction by CMS, and the only facility in the state to do so. The CMH difference is our commitment to the community, our patients, and each other. I am proud of this team and our accomplishments.

Brand New Emergency Department Opens
Community Memorial (CMH) is proud to announce that the renovation the Emergency Department has been completed and opened for patient care in late December 2021. The new Emergency Department (ED) at CMH in Hamilton has an expanded waiting room, private treatment spaces for patients, and enhanced safety features.
Patients needing emergent care can come to the new ED, located in the front of the hospital campus, through its own entrance. The expansion includes three additional rooms, increasing patient privacy and safety. The ED also has an added ambulance bay, and updated technology, including a centralized telemetry system to monitor patient cardiac status and security features that restrict entrance to the ED to those with permitted access.
As the pandemic continues to impact our community, access to safe, quality healthcare matters most. “The opening of the Emergency Department comes at an important time as we continue to serve the needs of residents in our community during the pandemic,” stated Jeffery Coakley, President and CEO of Community Memorial.
The continuous change in COVID, influenza and RSV cases are driving an increase in patient visits, and this expanded space allows for the care of patients more effectively and provides EMS and staff with the space and tools they need to deliver quality care to patients. With substantially lower wait times, prompt access to specialty services and seamless transition to tertiary facilities, the new ER at Community Memorial is the go-to for the community when emergent care is necessary.
“Providing vital care in personal, confidential rooms with increased space allows us to align our outstanding care with our facility,” stated Bernadette Casscles, RN, Director of Nursing Emergency Services. “The enhancements made to our Emergency Department reflect the 5-star awarded care we work to provide to our community every day.”
CMH has added a decontamination room with private access through the outdoor showers. This will allow patients, EMS personnel and staff to rinse off any potentially hazardous chemicals before entering the building. The decontamination room has an ante room to give staff the space they need to safely don and doff personal protective equipment and minimize the transfer of any hazardous waste or airborne diseases with an enhanced HVAC system. The opening of the Emergency Department wraps up Phase II of Community Memorial’s capital project, the largest transformational project since its inception in 1952. The $31m project will enhance the outdated facility, promoting patient access and navigation. Community Memorial opened a newly renovated Med/Surg Inpatient wing in January 2021, completing Phase I of the project. Welcome to the Team
Heather Bernard, DPN, RN, CIC, FAPIC, joined the Community Memorial Senior Leadership Team in February as the Vice President of Nursing and Clinical Services, filling the role of Diane Chase, RN, who entered retirement after 33 years at Community Memorial. Providing mentorship and guidance to nursing leaders and assigned patient care departments, Bernard is responsible for enhancing clinical outcomes, patient satisfaction and improving quality and patient safety. Previously serving as a Regional Infection Control Specialist in New York State with the Centers for Disease Control and Prevention (CDC) Foundation, Bernard is a long-time Hamilton resident and actually started her career in nursing at CMH 25 years ago. “I am proud to be back at CMH, where my journey began and continue its outstanding reputation for patient care,” Bernard commented. “My career journey and experiences have shaped my transformational leadership style. I look forward to supporting the provision of high-quality, evidence-based healthcare to our community.”
New Physicians

Expanding the current team of CMH hospitalists compiled of Nathan Keever, DO; April Lamunion FNP; and Lynette Champagne, NP; Dr. Hamad and Dr. McKay will round out the group of inpatient physicians and nurse practitioners who work exclusively inside the hospital. These providers care for patients admitted to the organization, collaborating amongst other departments during patient admissions, post-surgical care and swing bed patients.
Hamad studied medicine at the Alexandria University Faculty of Medicine in Alexandria, Egypt, where he received his Bachelors of Internal Medicine and General Surgery, before studying General Surgery and Etymology at Trafford Hospital in Manchester, United Kingdom. He received his Boards in Family Medicine in 2013 and Fellowship of Anti-Aging and Regenerative Medicine in 2016. Dr. Hamad has been providing care to patients in Central New York since 2010. He brings experience in inpatient management, integrative medicine, wound care, minor surgeries and rapid response/code management. His experience adds to the high quality of care patients expect from CMH’s healthcare system.
McKay comes to Community Memorial after an extensive career as a local physician. Graduating from St. George’s University School of Medicine and completing his family medicine residency at Mountainside Hospital, McKay was a private practice primary care provider, and has served as a hospitalist in a neighboring facility since 2018 as well. An Oneida native, McKay served as the President and Vice President of a medical staff group and the Chairperson of the Department of Internal Medicine. “Dr. McKay and Dr. Hamad bring experience a wealth of knowledge from larger health systems that will benefit patients in Hamilton without having to travel to other hospitals,” noted Jeff Coakley, President and CEO. “We’re excited for the expansion of skill that these providers bring to our Hospitalist Team.”

Nurse Practitioners Deliver Exceptional Care at Community Memorial
With healthcare at the forefront, patients are engaging more than ever with their primary care providers, and the Family Health Centers of Community Memorial Hospital (CMH) are seeing an influx of outpatient visits.
To both accommodate the volume of patients needing routine well-checks and physicals and also sick visits, nurse practitioners (NPs) have become increasingly common as primary care providers in many settings. Several factors have attributed to this, as many registered nurses further their education and careers by advancing to the next level as a nurse practitioner. “Working under the supervision of a physician, there are many benefits of seeing an NP for primary care needs,” states Dr. Robert Delorme, Chief of Family Medicine at CMH. “There is a common misconception that an NP could provide a lower level of care, however, in my experience, NPs are really the backbone of the primary care system and have a heightened sense of compassion and skill.”
Working as an NP in the primary care field for ten years, Cindy Marshall knows the career development aspect personally and the reason behind her work. “I started as an LPN and went back to school to complete the Nurse Practitioner program. Now, working in family medicine as a provider, I love the opportunity to help patients in multiple areas of their health,” states Marshall. “Emotionally, mentally and physically – all important ways we care for patients, especially in our rural setting. The personal relationship with my patients is what drives the improved care provided.”
Physicians overseeing NPS provide a collaborative environment for patient care and additional attention to detail for more complex cases. There is a seamless methodology that enhances the care patients receive. “Nurse practitioners are as capable as physicians, especially for the more common diagnoses, and because they’ve been in the medical field for so long as registered nurses they bring such a vast strength in more than just clinical care,” adds Dr. Delorme. “I’ve been a family medicine doctor for more than 40 years, I’ve personally worked with and supervised outstanding NPs. I feel the patient care and satisfaction provided by them is incredible.”
Gwen Manley adds why becoming a Nurse Practitioner was about more for her than just a career move. “I wanted to care for patients in an impactful way, improving their health but also their lives,” states Manley. “NPs provide individualized, all-encompassing care to all ages and populations.”

Community Memorial Announces New Cardiologist
The Community Memorial Hospital (CMH) healthcare system is excited to welcome a new provider to the team as Dr. Avneet Singh will join as the organization’s new Cardiologist.
Dr. Singh studied medicine at Maulana Azad Medical College and graduated with his Bachelor of Medicine and Bachelor of Surgery in New Delhi, India. Singh also completed education around internal medicine before completing his residency and fellowship in cardiology at SUNY Upstate Medical University. The previous Director of Echocardiography at Upstate, Dr. Singh received his certifications in both Cardiovascular Computed Tomography and Cardiovascular Disease. “Dr. Singh brings exceptional skill to our team, and his expertise will allow us to continue to provide high quality, patient-centered care throughout the Community Memorial network,” states Dr. Michael Walsh, Chief Medical Officer.
Dr. Singh comes to the CMH medical staff as longtime Cardiologist Dr. Raymond Carlson retires. Practicing in Hamilton since 2010, Dr. Carlson has built a strong reputation surrounding the cardiology care provided locally. “Dr. Carlson is certainly leaving a legacy behind as he enters retirement. Our medical staff appreciates his contributions beyond measure, as he’s been such a dynamic part of our team, and we’re eager to continue the quality of cardiac care to our patients with the addition of Dr. Singh,” adds Walsh.
Residing in Manlius. Dr. Singh enjoys playing tennis, cooking, hiking, practicing yoga, and spending time with family. “I look forward to being a part of a close- knit community of providers, and providing state-of- the-art, personalized cardiology care to patients,” adds Dr. Singh.
Dr. Singh holds professional memberships within the Fellowship of The American College of Cardiology, the National Board of Echocardiography, along with the American Heart Association and the American College of Physicians. He will be seeing patients out of the Hamilton Family Health Center location.

New Nurse Practitioner Joins CMH Team
The Family Health Center Network of Community Memorial Hospital (CMH) has welcomed another primary care provider to the team; Cassandra Senif, NP, is now seeing patients out of the Morrisville office location. Graduating with her Bachelor of Science in Nursing from Utica College, Senif continued to her

Master of Science in Nursing from SUNY Polytechnic Institute. Having worked in healthcare services for over a decade, Senif has experience in a range of patient care areas, from pediatrics and up. “Cassandra is a great addition to the Community Memorial medical staff team, covering a vast area of medical services in her clinical rotations and work history,” states Dr. Michael Walsh, Chief Medical Officer. “We enthusiastically welcome her and her skills, and are excited to watch her patient base grow as a CMH team member.”
Senif also holds certification in Emergency Nursing Pediatrics, along with the Pediatric Nursing Certification. “I’m excited to join such a great team of healthcare providers, and I look forward to welcoming patients in under my care,” states Senif. “Community Memorial is a stellar place to continue my career in healthcare.”
A resident of Hamilton, Senif looks forward to expanding her care to the community. “My area of interest as a Nurse Practitioner is treating the whole family; I have a significant background in pediatrics so I am comfortable treating both the child and the parents. I have completed my clinical training hours at CMH Hamilton and I am looking forward to continuing to learn from other providers and to build relationships both professionally with my peers and with my patients.” Senif enjoys running and hiking with her dogs and husband, as well as attending local college athletic events in her spare time.

Community Memorial Hospital: Building Today. Tomorrow. Always.
Community Memorial Hospital (CMH) opened its doors to patients in 1952 with a small Emergency Department, Operating Room, and an Obstetrics Unit. In that era, the facility met the basic healthcare needs of the community. Nearly 70 years later, the organization has grown, added service lines and medical staff, and works to adapt to meet the continuously evolving scope of healthcare. Even after the Emergency Department was expanded slightly in the ’80s, as were the Operating Rooms – the facility has become outdated, inefficient, and aged gracefully. Now, approaching its 70th anniversary, Community Memorial is undergoing a transformational project, set to update and renovate over half the facility.
Years of extensive planning and preparation came to fruition as construction began in June of 2020, in the vacant former Skilled Nursing Wing. As Phase I was completed in January of 2021, the brand new wing of all private inpatient rooms opened, serving patients immediately, at the height of a COVID surge. Phase II began with limited impact on patient care, as the Emergency Department was moved to the back of the building, allowing for the new, expanded ER to be renovated. “It was vital to maintain access for patients for emergency care services,” remarked Jeff Coakley, President, and CEO. “Relocating the ER temporarily kept our construction on schedule, but more importantly, allowed a critical service to stay available for every patient that needed it.”
Phase II is nearing completion and will elevate the care CMH provides to the communities that depend on it. As
connecting patients to important services is a key part of the CMH future, the leadership team and governing board have been in continuous review of further enhancing the organization, aligning the timing of funding available with the current construction work. “Our facility required many updates, both infrastructure, and technology-related, to remain viable,” added Coakley. “Our project scope has expanded, and we’re excited to now have new upgrades including a new MRI machine, which will further enhance the patient care provided right here in Hamilton. This unit will replace our 14-year-old, refurbished machine, and provide higher-level medical imaging, in a more efficient manner.”
The capital project will also aid the organization in additional ways, including strengthening existing service lines with increased space and enhancing the environment of care as departments are more conveniently located, allowing for better patient workflow and more efficient patient care. Consolidating spaces while utilizing previously vacant spaces will also encourage the CMH scope of services to be broadened.
“Every detail of the largest transformational project in our history has been meticulously reviewed with the staff, who will be using the spaces each day as they care for patients,” added Coakley. “It was imperative that this entire process fits the needs of our patients, but also our employees and medical staff, as they’re an integral part of this project, delivering exceptional care.”
With a projected completion in early 2023, the organization will have new departments for Laboratory, Rehabilitation, Radiology – and more – to join the completed Med-Surg wing and Emergency Department. This $31.5m capital project will allow CMH to remain competitive as the market for healthcare providers is increasingly challenging. “We’re excited to further develop our recruitment capabilities, as this new facility will also allow for better retention of staff,” noted Coakley.

CEO UPDATE
As 2021 comes to a close, I’d like to reflect on the tremendous work of the medical staff and every employee on the Community Memorial team as the COVID-19 pandemic continues to present challenges to “normal” life. It has been difficult for patients and families to navigate health services, which has escalated the anxiety and concern in our healthcare environments for patients and staff. Thank you to the patients and staff for their tolerance and compassion as our community works together to continue to support safe, high quality local health services.
Looking forward, Community Memorial is excited to be nearing the completion of our renovated and expanded Emergency Department. Welcoming entrances for ambulance and walk-in patients will be clearly marked and staff will be immediately available to assist community members with their medical emergency. Our Emergency Department providers and staff will provide your family a gateway to our own medical experts as well as the advanced services at other hospitals in our region.
We continue to renew critical health services including cardiology and primary care. In October, we welcomed A.V. Singh, M.D. as our new cardiologist and thanked Dr. Ray Carlson and his wife, Cyndi, for their many years of service in our community. We are also continuing to improve access to our growing primary care service and welcome Cassandra Senif, NP, to our network of Family Health Centers. We are encouraging community members to establish with a primary care provider if you do not actively see a provider at this time. Anyone that may have missed a routine well-check with their provider over the past year is encouraged to call today and reschedule any missed appointments.

Women’s Health at CMH
One thing is certain in all health issues – early detection is key, leading to more positive outcomes in the fight against a variety of illnesses. October is Breast Cancer Awareness Month, focusing on educating and encouraging patients to seek preventative care and vital screenings.
The Community Memorial Hospital (CMH) network offers primary care in five office locations, with a team of medical providers available for women’s health services, from routine well-woman exams, birth control options through menopause care. “It’s especially important for women to maintain a consistent lifestyle surrounding their healthcare,” notes Dr. Michael Walsh, Chief Medical Officer at Community Memorial. “As the cornerstone of a family’s overall health, their positive well-being promotes improved health for children and families, but also – women can face unique health issues and it’s vital to have regular check-ups.”
Breast and ovarian cancer rank among the top most common health issues for women. Bringing awareness to these issues, along with mental health issues that may then coincide with a cancer diagnosis, is another reason why Community Memorial encourages appropriate medical screenings for patients. Offering 3D Mammography services and breast ultrasound care is just one part of the Community Memorial initiative for women’s well-being. “There is no ‘one size fits all’approach to women’s health care,” adds Dr. Walsh. “With a focus on the patient’s health entirety, our team of medical providers can advise women on gender-specific screenings, based on a number of factors. Age, medical history, and genetics all come into consideration.”
Unlike larger, urban hospital settings, patients at Community Memorial can access medical imaging appointments quickly. The streamlined approach from a Family Health Center location to the Radiology Department at CMH allows for a prompt appointment for those vital mammograms. And, as a Crouse Health partner, patients enjoy the ease of navigation should they need a breast specialist to consult or follow up. “With regular, routine scans, the chances of early detection are increased, resulting in more patient treatment options,” adds Dr. Walsh.

Dr. Raymond Carlson is retiring after more than a decade of providing cardiology care for the Hamilton region and beyond. Dr. Carlson has been instrumental in cardiac care for patients admitted to the hospital, patients served in the office setting for consults and follow-up care, but also as a community contributor in the effort to educate and encourage patients to improve their heart health.

November is Lung Cancer Awareness Month
Lung cancer causes the most cancer deaths worldwide, accounting for 1.8 million new cases and 1.6 million deaths annually. In 1987, it surpassed breast cancer to become the leading cause of cancer deaths in women. Early detection and awareness can help save lives. Smoking is the most common cause of lung cancer. National Handwashing Awareness Week is the first full week of December and serves to emphasize the significant role that hands play in the spread of infectious diseases. The aim is to teach the importance of handwashing, hand hygiene, and mindfulness to protect the health of individuals and communities. Through participating in this event, we can reduce the spread of infectious diseases to improve our health and prevent a seasonal epidemic in our communities.
As we head into flu season, remember to keep preventive health care at the top of your to-do list. The Centers for Disease Control and Prevention recommends the annual influenza vaccine for everyone six months of age and older.
During National Influenza Vaccination Week (December 6-12), get your annual flu vaccine through your CMH primary care provider.
