Visiting Hours Suspended
EFFECTIVE THURSDAY, DECEMBER 9, 2021
Due to the increase in local COVID-19 positivity, Community Memorial Hospital has temporarily suspended visitation in the hospital, with the exception of emergent, end-of-life care, minors and patients with special needs or circumstances.

Warren Perry, Director of Surgical Services, Achieves Success in the Operating room
Warren Perry, BSN, RN, RNFA, CNOR, Director of Surgical Services, recently achieved success in the operating rooms for CMH after delays were experienced, due to holes in wrapped instrument trays for patient surgery.
Working with a vendor, Perry streamlined the sterilization process of Operating Room instruments, shifting the organization to a new process that sterilizes with steam in a sealed container. Reducing delays in surgery has improved patient safety as well as operational efficiency.
Mr. Perry explained, “For all of us in healthcare, the primary concern is the patient’s well-being. It’s our responsibility to eliminate as many variables and prevent as many infections as possible. And, this new process for CMH improves the patient care and experience, adding to our 5-star care in Hamilton.”

An Update To Visiting Hours
EFFECTIVE MONDAY, SEPTEMBER 13, 2021
Visiting hours will be strictly enforced. Exceptions may be allowed on a case by case basis with leadership approval.
Visiting hours will be as follows for Med/Surg and SCU:
- Monday through Friday: 3pm – 7pm
- Saturday & Sunday: 1pm – 4pm
- Each patient will only be allowed a maximum of TWO visitors per day.
- NO visitors in Amb/Surg.
- Visitors must maintain social distancing of six feet, even in patients rooms.
- Children under the age of 12 will not be allowed to visit.
- Visitors are not permitted to visit and patient who is in isolation, unless exceptional circumstances and prior approval have been obtained.
- Visitors are to remain in the patient room at all times unless directed by staff to leave due to a treatment.
CMH Visiting Hours Extended
EFFECTIVE FRIDAY, JULY 9, 2021

Temporary Policy Update for Visitation
EFFECTIVE THURSDAY, APRIL 1, 2021
Each admitted INPATIENT will be allowed TWO visitors at a time PER DAY at the hours of:
- 4pm – 7pm Monday – Friday
- 1pm – 3pm Weekends
All visitors will be screened for illness, including temperature. Masks are required for entry.
Guidelines are subject to change based on NYS Department of Health guidelines and any change in COVID numbers locally.
Community Memorial is pleased to announce 2 Community COVID-19 Vaccine distributions
Community Memorial is pleased to announce 2 COMMUNITY COVID-19 Vaccine distributions, scheduled for Tuesday, March 9th and Wednesday, March 10th. Both PODs (point of distribution) will be held at the Masonic Lodge in Hamilton, on Wes Road.
Community Memorial was able to receive 500 doses of the Johnson & Johnson single-dose vaccine. Those eligible at this time are Community Memorial hospital inpatients 65+, 18-64 with comorbidities, non-hospital 65+ years of age. Community members interested in vaccination are encouraged to review criteria and signup for an appointment time at the below links which will be opened for scheduling on March 7, 2021.
A collaboration between Community Memorial, the Madison County Department of Health and Colgate University has allowed the successful scheduling of these vital PODs, in a concentrated effort around public vaccination and the fight against the COVID virus. Any community members that have questions around the COVID vaccine or their personal vaccination need should consult with their primary care provider.
Community Memorial will continue to keep area residents informed around additional future vaccine distribution. In the meantime, the healthcare organization and its medical experts continue to encourage safe habits; hand washing, social distancing and wearing masks.

Community Memorial Announces New Chief Executive Officer
The Board of Directors of Community Memorial Hospital have named Jeffery Coakley, MBA, as the new President and CEO of the organization, effective November 4. Mr. Coakley will be transitioning to Community Memorial and filling the spot previously held by Sean Fadale, who left the organization in September to take a leadership position for another NYS hospital.
Community Memorial Board of Directors Chairperson, Eve Ann Shwartz, announced Mr. Coakley’s acceptance of the position. “I am delighted to welcome Jeff Coakley as our new President and CEO. The Board of Directors is excited to have such a qualified candidate fill this position. Mr. Coakley brings a strong background of professional hospital leadership experience, and made an unquestionably positive impression on the Board, employees and medical staff of our facility. We are all confident that he will further the successes that Community Memorial has achieved in patient care, while guiding CMH on the path to long term financial sustainability.”
Mr. Coakley comes from Oswego Health, where he most recently served as Chief Operating Officer, overseeing business development, growth of the health system including community relations and strategic planning. Mr. Coakley launched new programs while enhancing existing services, working to better serve the communities within its reach. Building a positive culture for physicians and staff to create a healthy working environment, which supported the organization’s ability to meet patient needs while remaining competitive in the market was his focus. Mr. Coakley also led the development of new medical centers, generating outpatient growth and financial surpluses for the organization.

Having a deep background in Healthcare Administration, Mr. Coakley brings a wealth of knowledge surrounding business development, strategic services and physician relations. He has experience developing programs and services offered to patients, but also in recruiting healthcare providers. As COO for Oswego Health, he led a multi-disciplinary team to develop a successful six-provider orthopedic practice, securing a critical service line for patients in the community. Additionally, he has experience in capital project transformations, overseeing the building of a $17m behavioral health facility, improving local access to a vital service.
Mr. Coakley is originally from Fulton, NY, and had been at Oswego Health for 23 years. He holds a bachelor’s degree from Oswego State University and earned his Masters of Business Administration from Le Moyne College. He also has completed the Leadership Advancement program offered through Cornell University/Healthcare Association of NYS Academy, as was named a Rural Healthcare Worker of the Year in 2013 by the NYS Association for Rural Health. In his new role at Community Memorial, he will be responsible for the overall leadership of the organization; all aspects of the strategic, operational and financial performance. Additionally, assuring the organization and its employees effectively perform in fulfilling the mission and goals, as the lead champion for clinical quality and service to every patient cared for.
Jeff currently lives in Fulton with his wife Gretchen, and plans to relocate closer to Hamilton. He and his wife have two adult daughters, Amelia and Olivia.
